- **Amendment 4** The purpose of this amendment is to include the Site Visit Notes from June 18, 2020. Closing date and time for this opportunity remains the same-Tuesday, June 23, 2020 at 1:00 PM local (Mountain) time. **Amendment 3** The purpose of this amendment is to change the date for the site visit, due to the amount of snow this area received over the past weekend.� The site visit will be on Thursday, June, 18, 2020.� Meet at the Ephraim District Office, 540 N Main, Ephraim, UT 84627 at 10:00 AM.� **You must contact Daniel Luke NO LATER THAN Wednesday, June 17, 2020 at 435-299-2367 if you plan on attending the site visit** You will need 4WD, ATV or UTV in order access the site. Closing date and time for this opportunity is extended to Tuesday, June 23, 2020 at 1:00 PM local time **Amendment 2** �The purpose of this amendment is to announce that there will be a site visit for this project on Friday, June 12, 2020.� Meet at the Ephraim District Office, 540 N Main, Ephraim, UT 84627 at 10:00 AM.� **You must contact Daniel Luke NO LATER THAN Thursday, June 11, 2020 at 435-299-2637 if you plan on attending the site visit.** You will need 4WD, ATV or UTV in order to access the site. Closing date and time for this opportunity is extended to Thursday, June 18, 2020 at 1:00 PM local time. **Amendment 1** The purpose of this amendment is to incorporate FAR 52.228-15-Performance and Payment Bonds (Deviation APR 2020) Not requiring hard copy bonding forms with the seal. �Electronic Bonding forms may be submitted. The Manti-LaSal National Forest anticipates a crushing and minor pit development project. The project consists of: Minor pit development, crushing 16,000 CY (Cubic Yards) of 2 inch minus pit run aggregate and placing it in a stockpile within the site. Material will be taken in even layers up to 3 ft. in depth across entire areas within the pit boundary as marked on ground by the Forest Service. The work shall be in accordance with the FP-14 �Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects�. Aggregate will be pit run material crushed to pass 2 in. Screen. The project location is: Project is located approximately 22 miles northeast of Ephraim, You can access the Horseshoe Flat Pit from Ephraim by traveling east on Highway 29 up Ephraim Canyon to the intersection with Forest road 50150 Skyline drive, then North on 50150 Skyline Drive to the pit.� Pit is located just East of 50150 Skyline Drive. A formal site visit will not be held.� If prospective bidders are interested in a site visit, please contact the project engineer, Daniel Luke at daniel.luke@usda.gov or 435.299.2637 Per FAR 36.204 the estimated price range is estimated to be between $100,000 and $250,000 This project is a total Small Business Set Aside. All future information about this acquisition, including amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition.
