Page 2 of 2 NOTICE OF INTENT TO SOLE SOURCE The Department of Veterans Affairs, Network Contracting Office 15 intends to award a sole source contract under the authority of 41 U.S.C 253(c) (1) FAR 13.106-1, to LOMBART BROTHERS, INC. 5358 ROBIN HOOD RD NORFOLK VA 23513 Contractor shall provide the following equipment and installation to the Columbia MO VAMC: Equipment Qty Haag Streit 900 BQ LED Slit lamp with Applanation tonometer 16 Reichert 11636 Minus Cylinder phoropter 16 Heine Omega 500 LEDHQ 18 Lombart Full Diameter Trial Lens set with plus/minus cylinders 7 Lombart CVS Visual Acuity system with front surface mirror set 16 Reichert AVIA Tonopen 23 Reliance FX-920-L Chair with Reliance 7900-IC Floor unit with wheelchair modification and chair glide 6 Reliance 5346 Pneumatic stool 6 Marco 101 Lensometer 7 Marco HandyRef-K Palm Automatic Refractor/Keratometer 2 All items must be onsite no later than September 1, 2020 for timeliness of new clinic activation. The contractor must be onsite to assist with installation of all equipment and all installation must be done in a way to not interfere with patient care. The NAICS code is 339112 with a size standard of 1000 employees. This action will result in a Firm-Fixed Price contract. This notice of intent is not a request for quotations; interested parties may express their interest by providing a capabilities statement NLT July 2, 2020 at 3:00 PM CST. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required pharmaceutical supply. When responding to this announcement, respondents should refer to Announcement, Notice of Intent. This notice of intent to award a sole source contract is not a request for competitive quotes.
