Description
PLEASE REFER TO ATTACHED COMBINED SYNOPSIS-SOLICITATION AND�PERFORMANCE WORK STATEMENT FOR ADDITIONAL DETAILS.�SUBMIT QUOTES USING THE ATTACHED EXCEL VENDOR PROPOSAL FORM. Contractor shall provide all labor, tools, supplies, and equipment to pick-up and dispose of biohazardous medical and pharmaceutical waste generated by the 10th Medical Group in accordance with (IAW) the United States Air Force Academy (USAFA) Biohazard Waste Disposal, Performance Work Statement (PWS) dated 11 June 2020. -�� CLIN 0001, Base, Period of Performance (POP) is 1 October 2020 - 30 September 2021. -�� CLIN 1001, Option Year One (OY1), POP: 1 October 2021 - 30 September 2022. -�� CLIN 2001, Option Year Two (OY2), POP: 1 October 2022 - 30 September 2023. -�� CLIN 3001, Option Year Three (OY3), POP: 1 October 2023 - 30 September 2024. Service shall be provided at the following locations and frequencies using the contractor provided containers specified. Pharmaceutical Pick-up and Disposal 1.�� Building 4102, Room 1003; Main Pharmacy: 2 x 18 gal containers; pick up once/month. 2.�� Building 5136, Room 104S; Community Center Pharmacy: 1 x 18 gal container; pick up once/month.* 3.�� Building 2355; Cadet Pharmacy: 1 x 18 gal container; pick up once/month. 4.�� Building 8118; Joint Refill Center: 8 x 18 gal containers; pick up once/month.* Biohazardous Pick-up and Disposal 1.�� Bldg 4104, Biohazard Waste Storage: 15 x 40-gal containers; pick up once/week.* 2.�� Bldg 2355, Cadet Medical Clinic: 1 x 40-gal container; pick up every other week.* 3.�� Bldg 2355, Dental Clinic: 1 x 40-gal container; pick up every other week.* 4.�� Bldg 2355, Rm 2P469, Biology Dept: 6 x 40-gal containers; pick up once/month.* 5.�� Bldg 5136; Community Center Health Clinic: pick up of one (1) to two (2) sharps containers once/month. 6.�� Bldg 9037, Veterinarian Clinic: 1 x 40-gal container; pick up once/month. 7.�� Bldg 2170, Sports Medicine Dept: 1 x 40-gal container; pick up once/month. 8.�� Option to utilize emergency pick-up as needed. A maximum of two (2)/quarter. Will occur during normal operating hours. �*� denotes �Mission-essential functions� that must be performed under all circumstances to achieve USAFA component missions or responsibilities. Failure to perform or sustain these functions has been determined to significantly affect USAFA's ability to provide vital services or exercise authority, direction, and control. The option to utilize emergency pick-up as needed. Emergency pick-ups will be a limited to a maximum of two (2) emergency pick-ups during each quarter. Any requests for emergency pick-up will occur during normal operating hours. Instructions for Quote The quoter shall submit the following company information on company letterhead to include the following: Solicitation number Contact name Email Address Company Address Telephone number Cage code DUNS number Business Size Monthly price, 12-month total price, and total quoted price for base plus three (3) year contract for service rounded to the whole dollar Certificate of Designation EPA Permit A statement confirming the Offeror is willing to hold its prices firm for at least 90 days from the date specified for receipt of offers Acknowledgement of any amendments made to the initial solicitation Points of contact (POCs) authorized to negotiate with the Government on the Offeror's behalf Use the attached pricing worksheet. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. The Synopsis-Solicitation is attached in full. PROVIDE PRICING FOR EACH CLIN WITH YOUR RESPONSE. SUBMIT QUOTES USING THE ATTACHED EXCEL VENDOR PROPOSAL FORM.
