All information received in response to this notice is marked as proprietary and will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The Butler Veterans Administration Medical Center (BVAMC) located at 353 N. Duffy Road, Butler, PA 16001 is performing market research to determine if there is a sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms (SB or other); or (4) large business (LB) firms to set aside a single award contract. The Government intends to award a single fixed price, Indefinite Delivery Indefinite Quantity (IDIQ) contract to utilize beds at an appropriate facility that will provide a personal care home (PCH) level of care as defined and licensed by the Pennsylvania Department of Human Services (PA-DHS). The Contractor must have a current and active license with PA-DHS and already be providing these services to single adults in order to compete for this contract. The Contractor shall be responsible for providing all components of the requirement necessary to provide BVAMC with community based residential care, treatment, and rehabilitative services for chronically mentally ill (CMI) veterans. Particular focus shall be on homeless Veterans. The residential contract beds are a vital part of the Department of Veterans Affairs Housing First initiative, which focuses on the continuum of care towards the goal of health care services and residential stability for Veterans. BVAMC is looking to contract for a total of three (3) contracted beds for single adult veterans who are either homeless or at imminent risk of becoming homeless, and who are clinically appropriate for a personal care home (PCH) level of care, or suspected of needing this level of care while undergoing further clinical assessment in this regard. See the attached DRAFT PWS for a full description of required services. The anticipated period of performance is five (5) years, with task orders to be issued as services are required. All interested firms who can meet the requirement should respond, in writing, and provide the following: 1. Describe your interest, including your capability to provide this service. 2. Indicate which socioeconomic business category for which you qualify. 3. Include the full address of the housing/facility and distance (in miles). 4. How long have you been performing these services? 5. Do you have an active license for that level of care Personal Care Home facility as licensed by the PA Department of Human Services? 6. What is the cost for a bed per day at your facility and what does this cost include? 7. How many beds do you have available at your facility? 8. Are you registered in the System for Award Management (SAM)? 9. What is your DUNS number? Provide this information to the Contracting Officer, Maureen Jordan at Maureen.Jordan@va.gov no later than 1:00 p.m. (Eastern Time), Monday, July 27, 2020. Solicitation 36C24420Q0835 BVAMC Transitional Housing (PCH) has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 624221 is applicable to this acquisition, with a Business Size Standard of $11.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract.
