Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06 dated 06/05/2020. This solicitation is set-aside for SDVOSB. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310 Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance, with a small business size standard of $8 Million. The FSC/PSC is J061 Maintenance and Repair of Equipment, Electric Ware, and Power Distributive Equipment. The Northern AZ VA Healthcare System in Prescott, Arizona is seeking to purchase transformer service, testing and repair. All interested companies shall provide quotations for the following: Services Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 Transformer service, testing and repairs (in accordance with Statement of Work) 1 JB Northern Arizona VA Medical Center (NAVAHCS) Prescott, Arizona Statement of Work CONTENTS: Project Objectives Statement of Work Requirements Special Technical Requirements Attachments PROJECT OBJECTIVES: Environmental Compliance Increased Longevity Reliability STATEMENT OF WORK/REQUIREMENTS: Contractor is to provide all services, tools, labor, equipment, and materials necessary to sample, test, and top off oil in 46 building pad mounted transformers (See Attachment A Transformer list). Remove and replace main power wires to building 151. Transformers shall be left energized while performing oil retrieval for testing. Oil shall be added to transformers as needed. Contractor shall install four new power cables from SCM 2 transformer to building 151 and add oil to each transformer as needed. Upon completion of installation, contractor shall provide two copies of oil testing results and oil added to transformer reports. Disposal of any and all hazardous material produced during this replacement project shall be the responsibility of the contractor. Work will be done Monday through Friday 07:30-16:00 except the following holidays: New Year s Day Martin Luther King Jr. s Birthday President s Day Memorial Day Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Day Christmas Day Time requirement to complete the project: Contractor shall make any attempt to complete the project within a minimum possible time. Contractor shall ensure they have all materials needed to complete the project without delay. D. SPECIAL TECHNICAL REQUIREMENTS: Drawing Files: N/A Meeting Minutes: If any meeting for this project is necessary, COR will maintain or produce meeting minutes. Codes: The following codes and criteria shall be utilized to the extent applicable. The contractor should have personal copies of these documents for use during the project. The contractor shall make the COR aware of any conflicts between these documents and VA Handbooks, as applicable to this project, and obtain a decision. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items (Jun 2020) FAR 52.212-3, Offerors Representations and Certifications Commercial Items (Jun 2020) Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. Additionally, all Service Disabled Veteran Owned Businesses who respond to this solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov by the end date/time of this solicitation. The following contract clauses apply to this acquisition: FAR 52.212-4, Contract Terms and Conditions Commercial Items (Oct 2018) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (Jul 2020) Please see complete listing of clauses and provisions (Attachment B) that apply to this RFQ. All quoters shall submit the following: verification of VA Form 10091 (Attachment C) and the W-9 (Attachment D) shall be sent directly to the paying office in Austin, TX unless the offeror has previously submitted these forms to VA Austin and provides certification of previous work. In all cases, the FMS Vendor File Update form (Attachment E) shall be submitted to the Contracting Officer as part of the offer or proposal. All quotes shall be sent to the contract specialist, Rochelle Bennett, at rochelle.bennett@va.gov. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Lowest price technically acceptable. The award will be made to the response most advantageous to the Government. Responses should contain your best terms and conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any. Submission of your response shall be received not later than 1500 AZT on Thursday, July 30, 2020 at rochelle.bennett@va.gov. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
