Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price contract in response to Request for Quotation (RFQ) IHS1405187. This solicitation is issued as a Small Business Set-Aside. (iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05. (iv) The associated North American Industry Classification System code is 238210 and the small business size standard is $16.5. (v) PRICE/COST SCHEDULE- Lowest Price Technically Acceptable �(vi) PURPOSE OF THE CONTRACT: See Attached SOW. (vii) Date and Place of Delivery: Start date: Date of award. Crownpoint Healthcare Facility, Crownpoint, New Mexico, 87313 (viii) PERFORMANCE WORK STATEMENT: See Attached SOW for Access Control System and Upgrade Video Surveillance System. (ix) Evaluation Factors: Evaluation will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.� The Government will determine the reasonableness of each acceptable offer. To be considered for award, your offer shall include the following documents: 1. Provide quote in the line items above. 2. Offered Services Specifications 3. Evidence that offered services meets the Technical Capabilities in Statement of Work.
