- REQUEST FOR INFORMATION/SOURCES SOUGHT NOTICE This is not a request for quotes; the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Network Contracting Office 8-San Juan, Puerto Rico is currently conducting a market survey for potential small business qualified firms capable of providing all management, supervision, personnel, services, protective clothing, materials, supplies, facilities, transportation, general and specialized equipment necessary to perform Life Safety Code survey of the buildings, update all Life Safety drawings, document deficiencies, identify appropriate occupancy classifications, and recommend corrective actions at the VA Caribbean Healthcare System (VACHS). The intent of this service is to ensure that the facility meets Life Safety Code (LSC) standards and has an updated Statement of Conditions as required by The Joint Commission (JC). The North American Industry Classification System (NAICS) is 541611. Responses to this notice shall include company/individual name, a service capability statement, examples of similar performances which you have provided services to, DUNS number, address, point of contact. Contractors should provide their size status for the above references NAICS code. (i.e. small, large) and whether they are certified SDVOSB, VOSB, Hub zone, 8(a), women-owned concern. Standard company brochures will not be considered sufficient response to this Source Sought. The Contractors Qualifications and Requirements are as follows: The Contractor must have 5 years of experience in Fire Protection Engineering Principles and must be a licensed professional engineer (PE). The Contractor shall provide documentation to reflect a current valid PE license in Fire Protection (any U.S. state/territory) with at least 5 years of continual, uninterrupted registration. The Contractor must have 5 years of experience conducting physical on-site life safety assessments. The Contractor shall provide a list of projects that involved on-site Life Safety Assessment using the National Fire Protection Association Code and the Joint Commission Standards and development of Basic Building Information, Egress analysis, Statement of Conditions, and Specific Plan for Improvement with the VA healthcare facilities. The list must include name of the facility/owner/owner representative and the project completion date. The Contractor must have 5 years of experience working with the current editions of the National Fire Protection Association and Standards. The Contractor shall provide a list of projects that involved design of Life Safety and Fire Protection drawings for healthcare occupancies using the National Fire Protection Association Code. The projects must reference assessment and development of Basic Building Information and Egress analysis. The list must include name of the facility/owner/owner representative and the project completion date. The Contractor must have 5 years of experience working with the Joint Commission Accreditation standards. The Contractor shall provide a list of projects that involved Life Safety Assessment with the National Fire Protection Association Code and the Joint Commission Standards and development of Basic Building Information, Egress analysis, Statement of Conditions, and Specific Plan for Improvement with the healthcare facilities. The list must include name of the facility/owner/owner representative and the project completion date. The Contractor conducting the assessment shall have current 2-hour Asbestos Hazard Emergency Response Act (AHERA) asbestos awareness training and half mask respirator fit test. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services under the same or a related NAICS codes. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. Do you plan on responding to a solicitation for this requirement with a Joint Venture utilizing multiple owned companies as majority or non-majority owner? Yes ___No ___ If yes, please identify which companies are considering a Joint Venture and the ownership of each company. If you re in GSA, please provide schedule and contract number. It is anticipated that the contract will be a Firm Fixed Price with a Base Period and Four Option Years of 12 months each.
