- Description
- The Department of Veterans Affairs established Health Care for Homeless Veterans (HCHV), in accordance with 38 U.S.C. 501, and 2031. Network Contracting Office 21 (NCO 21) plans to issue a Request for Quotes (RFQ) for Health Care for Homeless Veterans (HCHV) emergency housing that includes case management services on the Islands of Hawaii: Oahu, Kauai, City of�Hilo�and Island of Guam for Department of Veteran Affairs Pacific Islands Healthcare System.� The VA intends to issue a solicitation to result in a Single Award Indefinite Delivery Indefinite Quantity (IDIQ) contract with an ordering period not to exceed five years and six months.� This procurement will be solicited in accordance with FAR Part 12 Commercial Items and FAR Part 13.5 Simplified Acquisition Procedures.� This is paid on a per diem basis in accordance with Performance Work Statement. VA is obligated only to the extent authorized placement of patients are made under this contract. The program is designed to provide short term emergency housing with case management services to engage the Veteran(s) into the transition to permanent housing options. Each location described below will have the following separate estimated usage throughout a one (1) year performance period. Hilo, Hawaii����������������� five (5) beds per day for male Veterans Central Oahu: � ����������� five (5) beds per day for male Veterans Lihue, Kauai: ������������ � five (5) beds per day for male Veterans Mangilao, Guam: �������� five (5) beds per day for male Veterans Place of Performance: Islands of Hawaii: Oahu, Kauai, City of Hilo and Island of Guam.� The associated North American Industrial Classification System (NAICS) code for this procurement is 624221 with a small business size standard of $12,000,000.00. The FSC/PSC is G099. Contractors must be registered in the System for Award Management (SAM) at www.beta.sam.gov at the time of the RFQ response due date. ATTENTION VETERAN BUSINESSES: In order to be officially recognized by VA procurement activities as a Veteran-Owned Small Business (VOSB) or a Service-Disabled Veteran-Owned Small Business (SDVOSB), your firm must be verified in the VetBiz database at https://www.vip.vetbiz.va.gov/. The solicitation number for this RFQ is 36C26120Q0545.
