Description
A. Contractor shall completely prepare site; furnish labor and materials; and perform work for project: VA Consolidated Mail Outpatient Pharmacy (CMOP) Alternate Walkway; as required by drawings and specifications at the VA CMOP, Chelmsford, MA. Project includes all necessary work associated with the demolition of pavers and construction of new sidewalks. B. Place of performance: 1. Chelmsford CMOP - 10 Industrial Ave, Chelmsford, MA 01824 2. A site visit is scheduled for 13 August @ 7:30am. Please confirm your attendance with the Facility Manager, Mike Graybeal, at phone: (978)244-1311 or email: Mike.Graybeal@va.gov. 3. The location is at the front entrance on the NE side of building. The area labeled alternate walkway is approximately 19 x5 . 4. The facility is secure with restricted access. All work is performed on the exterior of the building. Parking and laydown area, if necessary, will be coordinated with the Facility Manager. C. The project includes: 1. Demolition of the existing pavers, grading, installation of a new sidewalk, and landscaping. 2. Contractor shall provide all labor, supplies, transportation, and equipment required to perform work. D. Work to be included but not limited to: The contractor shall provide a new sidewalk that complies with Massachusetts Architectural Access Board regulations (https://www.mass.gov/law-library/521-cmr) and ADA requirements. It shall be of similar construction to the adjacent existing ninety-degree sidewalk. After new sidewalk is installed apply stone borders on both sides of new sidewalk and weed barrier/edging. Contractor s Responsibilities: The contractor is responsible to provide a copy of their COI prior to work being performed. COI must list RAM-10 Industrial, LLC and Ram Management CO., LLC as additional insured. The contractor is responsible for verifying all site conditions prior to performing any work under this contract and follow all guidelines and standards. Before opening any excavation, it is the contractor s responsibility to verify the existence of underground utilities. Contractor shall verify all site dimensions prior to demolition and construction. The contractor is responsible for submitting a list of parts and materials to the COR for approval prior to use. To include: Concrete compressive strength Stone used Sealant used The contractor is responsible for protecting adjacent structures, equipment, vehicles, pedestrians, and vegetation while performing under this contract. The contractor is responsible for replacing concrete walkways, landscape, grass, asphalt, and any other damages caused by the contractor during this project. The contractor shall be responsible for the actions of all individuals provided to work under this contract. If in the event damages arise from work provided by contractor personnel, the contractor shall be responsible for all resources necessary to remedy the incident (including damages to landscaping) at no additional cost to the Government. The contractor shall adhere to all Occupational Safety and Health Administration (OSHA) regulations, as well as the safety regulations of the CMOP. The contractor shall ensure the worksite is properly cleaned up during construction and when work is complete. The contractor shall dispose of all residues off-site in strict accordance with all applicable Federal, State and Local regulations. The building entrance, which also serves as a fire exit, shall not be blocked during construction. Performance Schedule: Normal Working Hours: Normal working hours will be from 7:00 AM to 3:30 PM, Monday through Friday (excluding Federal holidays). Off-hour work may be required to accommodate special circumstances. Commencement of Work: The contractor shall commence work under this contract within ten (10) calendar days of receipt of the notice to proceed, or at the time of the kick-off/pre-construction meeting, whichever occurs later. Period of Performance: The contractor shall complete all work no later than thirty (30) calendar days of receipt of the Notice to Proceed. Contractor will provide Chelmsford CMOP a ten (10) day notice to start of work. The Estimated range to complete this work is less than $25,000.00. Pursuant to 38 USC 8127(d) and in accordance with Public Law 109-461, the Veterans Benefits, Health Care and Information Technology Act of 2006, this construction project will be solicited as a Total set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB). The North American Industry Classification Code (NAICS) for this procurement is 236220 (Commercial and Institutional Building Construction), with a small business size standard of $36.5 million. The duration of the project will be up to 30 days from Notice to Proceed. This completion period includes final inspection and cleanup of the premises. Award will be made utilizing the Best Value Lowest Price Technical Acceptable (LPTA) Source Selection Process in accordance with Federal Acquisition Regulation Part 15. Proposal submission instructions will be included in the solicitation package. The government plans to award without discussions. The offeror is responsible to monitor and download any amendments from the Federal Business Opportunities (http://www.fbo.gov ) website, which may be issued to this solicitation. In accordance with FAR 52.204-7, Required SAM and in accordance with Veterans Affairs Acquisition Regulation (VAAR) 802.101, VIP Registration, prospective contractors must be registered in SAM and in the VIP databases under the applicable NAICS. Firms may obtain SAM information at http://www.sam.gov and VIP information at https://www.vip.vetbiz.va.gov/ . The solicitation should be available for download on or about August 6, 2020 and the proposal due date will be on or about September 8, 2020. Only one site visit will be scheduled and interested offerors are highly encouraged to attend.
