- Page 1 of 3 The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with CUSTOM FABRICATING INDUSTRIES. to provide replacement and installation of window film in the canteen area for the Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 442291 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for Maintenance for MED GAS COMPRESSOR equipment for the Jesse Brown VA Federal Health Care Center to the contracting officer NLT 11:00AM CST, 10 Aug 2020. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with Doctor Oxygen STATEMENT OF WORK 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for opaque vinyl window film to be installed on the interior windows of the Canteen at the Jesse Brown VA. This requirement includes removing the existing window film, prepping the windows for the new film and installing the new film. There are 16 windows ranging in width from 2 -9 to 4 -4 with a total sq ft of approximately 210 sq ft of window area that shall be covered with the film. The film shall be 4 -10 in height and the film shall start 16 above the bottom window sill. The film shall cover 42 starting from 16 above the bottom sill to 4 -10 at the top. The film shall be delivered and installed at 2030 W. Taylor St.,Chicago, IL 60612. The installation shall be coordinated with Jennifer Podpora, Interior Designer at the Jesse Brown VA Medical Center. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and generation for final specifications. Window film delivery to the 2030 W. Taylor St., Chicago IL 60612 Duty hours will be during normal working hours 8:00am 3:30 pm weekdays. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new window film from the manufacturer until final acceptance by Jennifer Podpora, COR. Jennifer.podpora@va.gov Place window film on the 1st floor Atrium Canteen interior windows of the Jesse Brown VA Medical Center. Window film delivery and install will be scheduled through the Jesse Brown VA Interior Designer and COR, Jennifer Podpora. Provide maintenance manuals for the product specified. 3.0 KICKOFF MEETING Not Required 4.0 SUBMISSION OF QUESTIONS Questions: All questions must be received in writing, via e-mail, to the Contract Specialist before the closing date of the RFQ. All questions should be sent to XXXX, Contract Specialist at XXX@VA.GOV. Questions received after the stated date and time, may be addressed at the discretion of the Contract Specialist. 5.0 SUBMISSION OF QUOTATION Quote Submission: The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of: Volume 1: (technical) Complete manufacturer s product specifications Quantity of man hours for labor/installation Product literature Physical finish samples Warranty. Self-certifying statement confirming ability to meet project deadlines Volume 2: (pricing) Price quote with separate line for labor/installation Bill of Materials Submissions can be sent in multiple emails to avoid computer system email size limitation, to the GLAC, Contracting Officer. Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government s receipt of the contractor s quote. 6.0 EVALUATION CRITERIA Proposals will be evaluated on: Bill of Materials Cut sheets with product detail Labor/Services appropriate to the scope of work. (no pricing included) 7.0 PRODUCT Film type: Vinyl 2 mil Shade Fire Rating: Class A CSI: Division 8 Product will be free from defects in material and manufacture for a period of 5 years from the date of installation. Adhesive: Clear Pressure Sensitive General Image (Existing Windows and Film) 16 interior windows with the following width measurements: 2 -11 2 -9 3 -1 4 -4 4 -4 4 -4 4 -4 3 -0 3 -1 4 -4 4 -4 4 -4 4 -4 3 -1 2 -9 2 -11 8.0 SERVICES Installation: Wet installation that should be in accordance with manufacturer s installation instructions. Design Contract Tasks/Requirements The sub-contractor responsible for the installation of the vinyl window film shall attend one in person meeting with the Interior Designer, Jennifer Podpora to view the install location. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. c. All finishes shall be finalized prior to the order being put into production. This shall be coordinated with the Interior Designer. d. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. e. The subcontractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission.
