SOW Cardiac Mapping System 2 STATEMENT OF WORK (SOW) MEDICINE SERVICE/ CARDIOLOGY DIVISON DEPARTMENT OF VETERANS AFFAIRS THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A REQUEST FOR INFORMATION ONLY. This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Because this is a Request for Information announcement, no evaluation letters and/or results will be issued to the respondents. No solicitation exists. Therefore, do not request a copy of a solicitation. The Department of Veterans Affairs (VA), VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of meeting the following Brand Name Ensite Precision Cardiac Mapping System to be utilized at the CORPORAL MICHAEL J CRESCENZ VA MEDICAL CENTER, 3900 WOODLAND AVE, PHILADELPHIA, PA 19104 GENERAL DESCRIPTION: The Cardiac Electrophysiology Laboratory of Michael J Crescenz Medical Center is under the Cardiology Division of the Medicine Service line, directed by Dr. Andrew Epstein. The Cardiac Electrophysiology Physicians are licensed and credentialed to use advanced arrhythmia mapping technology to provide care to patients undergoing cardiac ablation procedures for management of arrhythmias. The equipment specific to this request is called the Ensite Precision Cardiac Mapping System by Abbott. The Ensite Precision is an advanced 3-D Mapping System that provides both contact and non-contact mapping that has an open-ended platform that supports the use of any ablation catheter. SUPPLIES/SERVICES The EnSite Precision Cardiac Mapping System is a device used to record cardiac electrical activity in order to create a dynamic, 3-Dimensional electroanatomic picture of the cardiac cycle. This technology is used to determine the precise origin of abnormal cardiac electrical activity that cause abnormal heart rhythms to support ablative therapy. SALIENT CHARACTERISTICS The Corporal Michael J. Crescenz Veterans Affairs Medical Center (CMC VAMC) Philadelphia are designed to outline all the CMCVAMC Cardiac Electrophysiology requirements. The specifications are provided below: The system must be compatible and integrated with the current Cardiac Electrophysiology Recording System, GE CardioLab, including integration with the system server hardware. The system must have an open-ended platform feature that allows for the use of almost any ablation catheter (flexibility). Provide contact and noncontact mapping The system must have intelligent automation tools with auto-map capabilities to reduce mapping time and produce high-reliability anatomic maps Turbo Map feature that allows the operator to rapidly map secondary arrhythmias Time detection algorithms that include last deflection Fraction and score mapping Guided auto-marking features for procedural consistency Customizable dashboards to guide lesion marking Stability monitor alerts that sense changes in heart rate, rhythm, respiratory changes and anatomy movement Ability to create 3-D models with a high level (CT Scan-like) detail and auto field scaling Provide fast, real-time contact force measurements of the ablation catheter Auto-map and One-map tools that provide simultaneous collection of anatomic and electrical points from multiple electrodes. Provide catheter cooling technology with ablative therapy STATEMENT OF WORK (SOW) Project Scope The Corporal Michael J. Crescenz Veterans Affairs Medical Center (CMCVAMC) is looking to purchase and expand our electrophysiology program using advanced cardiac mapping tools that integrate with our currently used technology (GE CardioLab, Siemens ARTIS Q X-Ray and Siemens SC2000 Ultrasound). The purchase must include end-user training for electrophysiology staff and physicians. Delivery Location CMCVAMC Philadelphia 3900 Woodland Avenue Philadelphia PA, 19104 Warranty and Support Warranty of the equipment must be provided for a minimum of one year upon receipt, acceptance and installation of equipment. The warranty must cover all hardware and software components of the Mapping System. Warranty and support should include a 4-hour call back response during normal business hours of 7:30AM- 4:30 PM Monday thru Friday (except holidays). 24-hour on-site response for repairs, service reports sent to all appropriate points of contact and one preventative maintenance visit prior to warranty expiration. The vendor shall provide and install all manufacturer recommended software upgrades and changes included in the cost of the system during the warranty/contract period. The vendor shall provide two (2) sets of user manuals and technical manuals to VA Biomedical Engineering and site end user. Repairs or services that are performed during normal working hours should be covered by warranty/contract. Installation All work and installation shall be coordinated with the department Nurse Manager, site Biomedical Engineering team and the Contracting Officer Representative (COR) and a phasing and work-schedule shall be provided. All work is to be executed as to not interfere or interfere as little as possible with normal functions of the Medical Center and department. The contractor shall keep work, storage and staging areas clean and neat. Testing The contractor shall verify to Biomedical Engineering and Electrophysiology Staff that the system meets all requirements through demonstration and validation. The contractor shall have no access to VA sensitive information during testing. Training The contractor shall be responsible for providing on-site user training for the cardiac mapping equipment. Each session of end-user training is to be scheduled at the CMC VAMC discretion and training shall have a one-year window. Work Hours Normal business hours are 7:30 AM to 4:30 PM Monday thru Friday excluding Federal Holidays. Work completed outside this time must be requested through Facility Biomedical Engineering. MISCELLANEOUS INFORMATION: Payment Net 30 days. The information identified above is intended to be descriptive, of the brand name Ensite Precision Cardiac Mapping System and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications mentioned above. Responses to this RFI should include company name, address, point of contact, phone number, and point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 334510 and 1250 Employees. Please answer the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer, distributor, or an equivalent solution to the items being referenced above? (4) If you are a large business, do you have any designated distributors? If so, please provide their company name, telephone, point of Contact and size status (if available). (4) (a) Describe how your small business takes ownership of the equipment in the manufacturing, assembly or delivery process. (5) If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (6) If you intend to subcontract any work on this contract, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to sub contracted work and completion of job. (7) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with any other federal contract? If so, please provide the contract number. (8) If you are an FSS GSA/NAC contract holder or other federal contract holder, are the items/solution you are providing information for available on your schedule/contract? (9) General pricing of your products/solution is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. (10) Please submit your capabilities in regard to the brand name equipment. (11) Please review salient characteristics/statement of work (if applicable) and provide feedback or suggestions. If none, please reply as N/A. (12) Please provide your DUNS number. This RFI will be conducted in accordance with Federal Acquisition Regulation (FAR) Part 13. Telephone responses will not be accepted. Responses must be received via e-mail to andrew.taylor3@va.gov no later than, 01:00 PM Eastern Standard Time (EST) on August 13, 2020. This notice will help the VA in determining available potential sources only. Do not contact VA Medical Center staff regarding this requirement, as they are not authorized to discuss this matter related to this procurement action. All questions will be addressed by the Contracting Specialist, Andrew Taylor. All firms responding to this Request for Information are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the FedBizOpps (FBO) web site for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services. The government will not pay for any information or administrative cost incurred in response to this Request for Information.
