- Description
- This solicitation has been amended to change the RFQ response date to September 9, 2020 at 3:30 p.m. PDT.� Reference the�attached SF-30,��Amendment No. 0001,��for details. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.� Solicitation No. 75H71320Q00030 is issued as a Request for Quotation (RFQ) for a firm fixed price commercial items purchase order.� The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) No. 2005-07, effective date 08/31/2020. The Portland Area Indian Health Service intends to solicit and negotiate on a single source basis with the Native American Rehabilitation Association, Inc., (NARA), 1776 SW Madison Street, Portland, Oregon, 97206-1715.� This single source procurement will be conducted under the authority of 41 United States Code (U.S.C.) 1901 as set forth in Federal Acquisition Regulation (FAR) 13.5.� Requirements Detail:� The Contractor shall provide youth residential treatment services to eligible American Indian & Alaska Native (AI/AN) male and female youth from Federally recognized tribes in Oregon and other states in accordance with the attached Statement of Work.� The facility shall provide residential care 24 hours a day, 7 days a week, and 365 day a year to assist youth to recover from complex substance use disorders and related consequences of problematic use. �The facility shall provide culturally competent and responsive care to youths, their families and communities, with the goal to improve the overall health care status and provide American Society of Addiction Medicine (ASAM) level 3 or higher.� Other services may be provided, including educational programming, medical and emergency referrals as needed, and referrals for additional mental health and other co-occurring disorders.� Youths generally present with complex trauma, significantly impairing mental health disorders, psychosocial difficulties, behavioral problems, and other conditions. The performance period will be for a one (1) base year period plus four (4) one-year options.� The performance period shall begin on September 28, 2020 through September 27, 2021. The Contractor shall provide firm fixed-pricing for youth residential treatment services for: Base year:� September 28, 2020 through September 27, 2021 Option Year 1:� September 28, 2021 through September 27, 2022 Option Year 2:� September 28, 2022 through September 27, 2023 Option Year 3:� September 28, 2023 through September 27, 2024 Option Year 4:� September 28, 2024 through September 27, 2025 Evaluation of options shall not obligate the Government to exercise the option(s).�� The NAICS code is 623220 and the small business size standard is $16.5 million. The Offeror shall include a completed copy of the FAR provision at 52.213-3, Offeror Representation s and Certifications � Commercial Items, with its offer. FAR clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. FAR clause at 52.214-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders � Commercial Items, applies to this acquisition. ATTACHMENTS: SF 1449 Statement of Work � Attachment 1 FAR and HHSAR Provisions and Clauses � Attachment 2 The basis for award will be to the responsible Offeror whose offer conforms to the solicitation and will be the most advantageous to the Government, price and other factors considered.� Other factors for this solicitation include qualification, key personnel and past performance of the Offeror.� Past performance information may be based upon the Government�s knowledge of and previous experience with the offeror, or other reasonable basis.� The award will be made to the responsible contractor pursuant to FAR subpart 9.1.� The Offeror must be registered in the System for Award Management (SAM).
