Description
Veterans Health Affairs, Network Contracting Office 5, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide Full Service/Preventive Maintenance Contract for Handicare Patient Ceiling Lifts including parts, on site labor, preventive maintenance, and visual inspections with load testing as described below that will not interrupt the continuum of care.  This requirement is defined primary by existing Patient Ceiling Lifts already in use at the Martinsburg WV Veterans Affairs Medical Center, located at 510 Butler Avenue . All responses to this Intent to Sole Source are due by 9/15/2020 2:00 PM EST. Type of Contract Needed: Annual one-time Full Service/Preventive Maintenance(with option years). General Description of need: Annual one-time Full Service/Preventive Maintenance Contract for all Handicare Patient Ceiling Lifts including parts, on site labor, preventive maintenance, and visual inspections with load testing. Location: V.A. Hospital 510 Butler Avenue, Martinsburg, W.V. ( Equipment listing and locations in the hospital on attached sheet). Specifications General: The contractor shall provide all supplies, materials, equipment, labor, supervision, management and transportation to perform all tasks as identified below. Scheduling: Hours of Work: All work shall be performed during 2nd shift business hours from 3:30 P.M-12:00 A.M., Monday through Friday, excluding federal holidays. The starting time period for service will be Oct. 1st, 2020 with a preference for option years included. Standards: Work to be accomplished to meet manufacturers specifications. Tasks: Adjust, lubricate, visually inspect, load test and repair as needed with specificity to the following items: -The lift lifting tape shows no sign of fraying or breaking along its entire length. -The stitching on the lift lifting tape where it connects to the carry bar shows no sign of fraying or breaking. -The airline tube that connects the hand control to the lift is not kinked, twisted, knotted, cut or damaged. -All the functions on the hand control work correctly(e.g. UP/DOWN/LEFT RIGHT. Etc.). -The brackets that hold the track in place on the ceiling are secure and tightened to manufacture specification. -There are no cuts, dents, or sharp edges on the carry bar that may damage the straps of the sling. -Ensure the lift makes no unusual sounds when the carry bar is moved UP/DOWN, or the lift is moved LEFT/RIGHT. -Ensure that there are end stops installed at each end of the track. -Ensure the lift moves freely along the entire length of the track. -Certify and record of lift inspection for inspector records. -Record then reset the P.M. lifts counter. -Inspect complete lift operation. Adjust track connections at gate transitions. -Inspect all power connections and record all power voltages. -Replace batteries and gate bearings, if the gate bearing is not functioning properly. -Lubricate moving parts as needed, gates, curves, motor. -Clean all rails and trolleys within rails. -Check strap limit switches. -Ensure the lift is up to current specifications and add anything that would fall outside of that. -Bring common parts to ensure downtime is limited if the motor needs additional work. Special Provisions: Ensure patient care is not interrupted. Interference to Normal Function: Contractor personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. Disposal of Hazardous Wastes: The contractor shall not dispose of any excess waste at any location on the medical facility premises except as specified by the contracting officer. Storage of Material: No materials or equipment shall be stored or kept at the Veterans Affairs Medical Center when the operator is not working, without the express approval of the Contracting Officer. Call Back Requirement: Response must NOT RESTRICT and be Justifiable. Routine: The Contractor shall within 12 hours after receipt of notification by the Contracting Officer or designee, and at no additional charge to the Government, repeat previously performed service when proved to be inadequate. Emergency: The Contractor shall within 4 hours after receipt of notification by the Contracting Officer or designee, and at no additional charge to the Government, perform Services to correct an emergent condition. Required Contractor Reporting: N/A Service Tickets: Contractor is responsible for supplying, completing, and submitting all reports required or requested by Federal, State, or local ordinances which pertain to any duties contained in the contract. The Contractor will be required to furnish the Contracting Officer, prior to performance of any work, the trade names (if any) and chemical names of all chemicals to be used and a label showing the contents, the use strength of the chemical as applied and the antidote thereto. Contractor will furnish the same information each time a change is made to the chemicals or products used in the performance of this contract. This information is required for emergency treatment in the event of ingestion and/or contact with the material by humans. Identification, Parking, Smoking, and VA Regulations: The Contractor s personnel shall abide by all VA Medical Station policies concerning badges, parking assignments, etc. The VA will not invalidate or make reimbursement for parking violations of the contactor personnel under any conditions. Smoking is prohibited on federal grounds. Possession of weapons is also prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations may result in citation answerable to the United States Federal District Court. General Conditions-N/A
