- This Request for Quote (RFQ) is for one (1) Nonpersonal Service Contract for Psychiatrist Physician to provide uninterrupted direct and telehealth patient care services to be performed in the Psychiatry Department at Northern Navajo Medical Center (NNMC) Shiprock, New Mexico. Psychiatrist Physician Service shall be performed in accordance with the attached Performance Work Statement (PWS). Period of Performance: Date of Award/Selection to a full year. The assignment are through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. NNMC will NOT consider candidates with sporadic availability dates. Candidate will work consecutively throughout the Period of Performance indicated. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours are only estimated. Work Schedule: Work Shift: The shifts hours are 8 hours from 8:00 a.m. 4:30 p.m., Monday through Friday including some weekends. Telehealth and some in house calls. CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. As a Psychiatry Physician, contractor shall provide prescribed medical treatment and personal care services to American Indian and Alaska Native beneficiaries. Duties include documenting observations, assessments, and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of competency standards. Our facility is seeking individuals who possess good organizational skills which enable completion of work, a demonstrated ability to assume leadership in a patient centered program, and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. Background Investigation: Selected qualified candidate will undergo FINGERPRINTING and CREDENTIALING process before commencement their tour of duty. Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. CREDENTIALING: Credentialing MUST be cleared and approved prior to contract assignment. Contract assignment(s) are conditional on a cleared and approved Credential. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for Physician services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant subfactors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the position and the PWS to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Factor 1: Candidates Past Performance, Must provide experience of comparable scope and complexity in providing Audiology Services within the past (3) years for the following; Subfactor: Outpatient Psychiatry Subfactor: Provide past experience in outpatient Psychiatry Subfactor: Preference for candidates able to provide full spectrum Psychiatry services Factor 2: Psychiatry Physicians Qualifications Requirements MUST BE PROVIDED. A complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information, Professional Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 6.0). Subfactor: License/Registration (Psychiatry Residency Trained, Board Certification or Board Eligible) Subfactor: Certification (BLS) Subfactor: DEA Registration License / Controlled Substance Subfactor: Must provide no history of drug or alcohol abuse Subfactor: Health Requirements (Medical Evaluation / Immunization) Medical Evaluation: The contractor shall possess a health status allowing physical and mental ability to carry out required functions. This is to be verified on an annual basis by the employing agency or a copy of a physical examination provided by the contractor/practitioner. Immunization: Contractor shall provide evidence of current immunization documentation for MMR (measles, Mumps, and Rubella) Diphtheria, Hepatitis B series, Varicella or positive titer, Tetanus Diphtheria, TD Mantoux skin test documentation and DT Booster, OOD status, or as required by the Navajo Area IHS immunization policy. A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Factor 3: Candidate Current / Valid Contact Information Subfactor: Physical Address, Phone Number & Email Address Subfactor: Provide Three (3) current references with active telephone numbers for candidate. Subfactor: Provide evidence of positive peer reference from last assignment Factor 4: Psychiatry Physicians Availability Subfactor: Upon fingerprinting clearance and credentialing clearance, physician shall be available as soon as possible to start tour of duty. Subfactor: Psychiatry Physician shall be flexible to allow for short term (13 week), where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. PROPOSAL SUBMISSION INSTRUCTIONS: 1. Quotation in the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). 2. Past Performance 3.
