- Attachment 1: Template for Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-07. This solicitation is a 100% Service Disabled Veteran Small Business (SDVOSB) set aside and only qualified contractors may submit quotes. The associated North American Industrial Classification System (NAICS) code for this procurement is 541910, with a small business size standard of $15 million. The FSC/PSC is R405. The VA Northern Indiana Health Care System, 1700 East 38th Street, Marion, IN 46953-4568 and 2121 Lake Ave, Fort Wayne, IN 46805 is seeking to purchase Real Time Wireless Public Restroom Customer Feedback Equipment and Monitoring Service. All interested companies shall provide quotations for the following: Supplies/Services Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 Real Time Wireless Customer Feedback Data Collection and Analysis Service for 40 Restrooms (Marion VAMC Campus) 12 MO 0002 Real Time Wireless Customer Feedback Data Collection and Analysis Service for 35 Restrooms (Fort Wayne VAMC Campus) 12 MO 0003 Installation of Data Collection Devices (Marion VAMC Campus) 40 JB 0004 Installation of Data Collection Devices (Fort Wayne VAMC Campus) 35 JB The Environmental Management Service Department of the Northern Indiana VA Health Care System requires technology that would measure customer satisfaction of 75 public restrooms in real time and receive customer feedback immediately. By providing real-time feedback to EMS personnel so as to ensure optimal cleaning performance for all public restrooms, reduce rework, provide safe facilities, and reduce custodial costs. 1) A small unit or system is required because some of the restrooms at out facility are small and will not accommodate a large system or unit. 2) Environmental Management Service Department requires a system that is ISO9000 certified. 3) The customer shall not need to text or call a number to provide feedback, the feedback from the customer must be collected by the unit. 4) The system or units must not require use of any VA Networks. 5) System must be able to alert designated users based on pre-set criteria such as SMS or e-mail. 6) The units or systems must be able to be cleaned and sanitized easily. 7) Data collected must be available in real-time and accessible from a secure web interface for designated users. 8) System must be scalable to be able to add additional Customer Feedback sites and feedback topic domains. 9) System must be capable to be administered from a secure web interface, and from this interface be able to access all data, run custom reports, and other analysis functions. 10) Data collected must be easily exportable using open data formats for easy import into third-party applications for further analysis. The contract period of performance is one year from date of installation, and installation shall be able to be provided no later than 60 days after receipt of order (ARO). Quote must be good for 90 calendar days after close of this Combined Synopsis Solicitation. All quotes shall be sent to the Network Contracting Office 10, at robert.sagris@va.gov Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Evaluation Factors: Award will be made to the offeror who represents the best value to the Government. Offers will be evaluated based on price, technical, past-performance, and delivery. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
