Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. �The solicitation number for this requirement is 00HCUGHD-2021-49849 and is hereby issued as a Request for Quotes (RFQ). The Centers for Disease Control and Prevention intends to award a FIRM-FIXED-PRICE contract for the requirement.� Please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options (if applicable). All services shall conform with the description as outlined in with the attached Performance-Based Work Statement. The objectives of this contract are to obtain spiked samples in biological matrices necessary for accomplishing ERB�s mission to maintain a proficiency testing program for toxic chemicals of interest in order to support the measurement of exposures to chemical threat agents. This requirement includes planning, producing, and distributing proficiency testing materials in support of ERB�s chemical emergency preparedness and response goals.� The primary purpose of this project is to obtain proficiency testing samples from an ISO 17034 provider to support proficiency testing activities for toxic chemicals of interest. The contractor shall produce a homogeneous lot for each proficiency testing event that meets the criteria listed in the Performance Work Statement (PWS). The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://www.acquisition.gov/browse/index/far. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2021-01 (Oct 1, 2020). This acquisition is full and open competition after exclusions of sources. This procurement has been designated as a small business set-aside for the Production and Distribution of Proficiency Testing Samples for LRN-C under North American Industry Classification System (NAICS) code 325998 and small business size standard 500 employees. A FIRM-FIXED-PRICE purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management. To register apply via the Internet at http://www.sam.gov.
