COMBINED SYNOPSIS/SOLICITATION LAUNDRY/GARBAGE CHUTE AND FIRE BARRIER INSPECTION/MAINTENANCE AT THE BUFFALO VAMC (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through email on RFQ reference number 36C24221Q0064. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-01. (iv) This solicitation is being issued as a competitive open-market SDVOSB total set-aside procurement. The North American Industry Classification System (NAICS) code is 541350, Building Inspection Services, with a small business size standard of $8 Million. (v) The Contractor shall provide pricing to provide monthly garbage/laundry chute services and annual inspections of all fire/smoke barriers at the Buffalo VAMC with a period of performance of 12/1/2020 to 11/30/2021 with the provision of four (4) option years. TECHNICAL Please provide a maximum 3-page capability statement that indicates the vendor has the trained personnel, equipment, and three years past performance record of working experience in laundry/garbage chute maintenance and fire barrier inspection/certification for healthcare, industrial, educational, and/or commercial facilities. PAST PERFORMANCE The Government shall request the contractor provide a list of contracts completed during the past three (3) years and all contracts and subcontracts currently in process for laundry/garbage chute maintenance and fire barrier inspections in healthcare, industrial, educational, and/or commercial facilities. Include the following information for each contract: a) Name and address of contracting activity b) Name and telephone number of contract person c) Dates of contract performance The Government reserves the right to make award without discussions. SITE VISIT The Contractor shall attend a mandatory site visit on 10:00am, Thursday, November 5th, 2020 at the Buffalo VA Medical Center, 3495 Bailey Avenue, Buffalo, NY 14215 in order to gauge the fire/smoke barrier quantity and view chute system. o Contractors shall email the Contract Specialist (Nathan.northrup@va.gov) no later than 4pm EST on November 4th, 2020 with their intention to attend. Contractors shall meet in the hospital lobby no later than 10:00am, Thursday, November 5th. (x) The provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items applies to this acquisition. * All firms or individuals submitting a quote shall include a completed copy of this provision OR have current Representations and Certifications in the System for Award Management (SAM - https://www.sam.gov). To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
