- Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. VHA Reginal Procurement Office (RPO) 2 is currently conducting a market survey for qualified firms capable of providing Visual Field Analyzer. The North American Industry Classification Systems (NAICS) is 339115. Responses to this notice must be submitted in writing (email or fax) and must be received no later than November 12, 2020 @ 10AM EST. Email: LATEISHA.ROBINSON@VA.GOV and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. Please review the attached Statement of Work attached herein. SOW: Visual Field Analyzer Scope of Work Brooklyn Surgery must replace the Visual Filed Analyzer in our eye clinics. The current one no longer functions and has been deemed by Biomed as no longer fixable. Eye and brain disorders can cause loss of peripheral vision. Visual field tests are performed by eye care professionals to detect blind spots and other visual field (peripheral vision defects). Optic nerve damage caused by a disease called glaucoma creates a very specific visual field defect. Other conditions associated with blind spots and other visual field defects include diseases of the retinal, brain tumors, and stroke. Objectives We have been notified by the National Surgery Office that we must have a cell saver in our operating rooms. Currently, when needed we call the Blood bank who comes in with a machine during a case. We are failing the National Surgery Office s audits because we do not own one of our own. The company will do the initial setup in the eye clinic. Place of Performance / Delivery The equipment is to be delivered to the warehouse/loading dock at the Brooklyn VA campus, and then delivered from the warehouse to the eye clinic in the new building. Below is the address: NYHHCS Brooklyn Campus Warehouse: 800 Poly Place Brooklyn, NY 11209 Travel/Parking The contractor will be responsible for their travel. Government-Furnished Property Contractor will not need any government furnished equipment or property. Education and Training Requirements There is no new education involved. This is replacing an analyzer that we are knowledgeable about and know how to use. VA Service Point of Contacts VA Service Point of Contacts Dr. Evan Canellos, Optometry Limited Source Justification/Recommended Source We do not have perfusionists at the Brooklyn VA to manage the cell saver during a case. The Blood bank comes in when needed. This is the call saver that the Blood Bank currently uses they are well versed in its use. VA Patient Privacy and Network Infrastructure None of the equipment listed within this procurement package will access the VA Network or store any sensitive patient information. Any contractors that will come on site for install will not need access to VA network. All contractors must comply with VA regulation and complete the following training: VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access or use to VA Sensitive Information. Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions. The system does not transmit or maintain electronic Protected Health Information. A Manufacturer Disclosure Statement for Medical Device Security (MDS2) form in not applicable. A VA Directive 6550 Pre-Procurement Assessment form is not applicable. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 339115. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
