Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. No Solicitation is currently available. All information contained in this Sources Sought Announcement is preliminary, as well as subject to modification, and is in no way binding on the Government. The Government will not pay for any information submitted in response to this request. The information requested will be used solely within the Department of Veterans Affairs to facilitate decision making and will not be disclosed outside of the Government. The determination of a procurement strategy, based upon the comments submitted in response to this Sources Sought Announcement, is solely within the discretion of the Government. The applicable North American Industry Classification System (NAICS) Code is 561210, Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance, and the small business size standard is $41.5M. Potential Offerors must be registered in the System for Award Management (SAM) to be eligible for an award (See Internet Site https://www.sam.gov/portal/public/SAM/). Potential Offerors must also have a current Online Representations and Certification Application on file with SAM. Service Disabled Veteran Owned and Veteran Owned Small Businesses must be registered and verified in the Department of Veterans Affairs Vendor Information Pages (VIP) (See Internet Site https://www.vip.vetbiz.gov/). Brief Description of Services This is a non-personal services contract to provide periodic services to Lumenis O.E.M (original equipment manufacturer) surgical lasers for the VA Southern Las Vegas Health Care System in North Las Vegas, NV. SCOPE. Lumenis is the original manufacturer of three surgical lasers owned by the VASNHS. Equipment Covered include: EE 40546 Equipment Description: Acupulst 30ST Laser Manufacturer: Lumenis Model#: Acupulse 30ST Serial#: 107 Acquisition Date: 3/19/2012 Location: VAMC Podiatry EE 53115 Equipment Description: Versapulse Powersuite 100 Laser Manufacturer: Lumenis Model#: VersaPulse Powersuite 100 Serial#: 492 Acquisition Date: 3/19/2012 Location: VAMC Surgery EE 40527 Manufacturer: Lumenis Inc Model # Selecta Duet Serial #: 10498 Acquisition Date: 3/19/2012 Location: VAMC Eye Clinic Specific Tasks. Service Technician Personnel: All equipment service related activities need to be performed by an individual who has successfully completed, by Lumenis standards, the Lumenis Comprehensive Laser Training courses; including but not limited to electrical safety training, laser safety training, and clinical training offered by a certified Lumenis clinical trainer. The technicians must be Lumenis trained and certified technicians. There are no exceptions. Service Hours: Lumenis Full-Service Contract Service is to be during normal business hours, M - F, 07:30AM to 4:00PM Pacific Time. Activities and Required Services Provided by Contractor Included in Contract costs: A full-service contract will include the following; Preventative Maintenance: Two PM visits will be included and scheduled with Biomedical Engineering. The PM s shall be conducted following manufacturer specified procedure. All costs for preventative maintenance parts are included. Parts provided must be the original manufacturer parts. Service Engineers will also recalibrate Equipment to ensure it conforms to Lumenis performance specifications. On-site repair service: including the cost of parts, travel and labor (During normal working hours 7:30am-4pm Pacific Time, Monday-Friday). Contractor shall provide service within 48 hours of initial on-site repair request. Technical support: Unlimited technical support is included Monday-Friday 6:00am-5:00pm Pacific Time. Updates: All software and hardware maintenance is included Loaner: Provide loaner equipment for extended repairs, based upon availability Service technicians are to provide certificates of competency. Service technicians are to check in with the Security Officer on Duty at the VAMC. Service technicians are to check in & out with Biomedical Engineering @ (702) 791-9000 x14062 upon arriving at the VAMC for performance of work. Service Report. Contractor shall provide a written service report for the Government records of repair. The report shall include the repairs provided, parts replaced, and any additional maintenance information deemed necessary by the manufacturer to ensure the highest level of operation. This report shall be forwarded electronically to vhalasbiomed@va.gov utilizing Microsoft suite software or PDF no later than five (5) days after repairs. Shipping. The contractor shall provide the required shipping materials (box and packing materials) to ensure the sensitive equipment is securely packed. These shipping material costs shall be included in this agreement. 5. PERIOD AND PLACE OF PERFORMANCE. Base year: 12/09/2020-12/08/2021 Place: VAMC, 6900 N. Pecos Rd., N. Include general information and technical background describing your firm's experience on contracts requiring similar efforts to meet the program objectives. (g) List of Projects (Government and Commercial) that are similar in scope and size. (h) Anticipated Teaming Arrangements (if any) (i) GSA Contract Number if applicable. (j) Copies of training certificates as identified above under Contractor Qualifications Interested parties shall not submit more than 10 single-sided, type-written pages using 12-point font or larger. Published literature may be any number of pages. The Government will use this information when determining its business type decision.
