COMBINED SYNOPSIS/SOLICITATION NOTICE 36C26221Q0119 Responses are due by 14:00 (PST), November 25, 2020 COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C26221Q0119 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as open market, set-aside Service-Disabled Veteran Owned Small Business (SDVOSB), NAICS Code is 541519, Other Computer Related Services. Place of Performance: VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822. The Department of Veterans Affairs, Network Contracting Office 22, VA Long Beach Healthcare System, is seeking sources that can provide a Restroom Cleanliness Customer Feedback Survey System for the VA Long Beach Healthcare System that meets the following characteristics: Product & Service Description: ITEM NO. DESCRIPTION QUANTITY UNIT OR STOCK NO. 18x REAL-TIME FEEDBACK DEVICE(S); 18 EA SMILEY BOX(S) WALL-MOUNT FOR SMILEY BOX(S) QUESTION PLACARDS 18x ATTENDANCE TRACKING DEVICE(S); 18 EA ATTENDANCE TRACKER WALL MOUNT WITH COVER Install and Set-up 1 EA Shipping and Handling (if applicable) 1 EA Salient Characteristics: COMPREHENSIVE SOFTWARE & SERVICES shall include; 12 Month Full Connectivity Unlimited toll-free support, five (5) days per week, 8:00am 5:00pm EST, Monday Friday without additional charge to the Government Preventative maintenance shall include monthly quality checks on all eighteen (18) Smiley Box(s) Survey Terminals Data communication and processing of feedback and all associated IT cost (i.e., server, database, License, IT security ) All report production in English for unlimited number of users Static data management (employee information, equipment location change, question change, etc.) Web access with real-time data Unlimited alerts by email/push notification to unlimited number of users Equipment monitoring report for client Mobile application for unlimited number of users SMS and email message alerts (widgets) Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition.
