Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Include DUN & Bradstreet number on all quotes. The Department of Veterans Affairs in Albany, NY needs to procure Medical Grade Gloves for VISN02. (ii) The combined synopsis solicitation number is 36C24221Q0214. It s issued as a Request for Quotation. (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2005-89 (7-14-2016). (iv) The combined synopsis solicitation is 100% Set-Aside for Small Business. The NAICS is 3339113; Business size standard is 750 employees. If NAICS code is different please supply that on your quote. Note, Interested Service-Disabled Veteran Owned Small Business (SDVOSB)/Veteran Owned Small Business (VOSB) Quoter must be listed and certified in Vetbiz.gov to be considered for Award. ALL requirements of this solicitation (Firm Fixed Price). (v) This combined synopsis solicitation is for the purchase of: Gloves, Medical, Nitrile 1. 1366 cases Small Gloves ~ 10 Boxes per case / 100 per box 2. 2805 cases Medium Gloves ~ 10 Boxes per case / 100 per box 3. 2375 cases Large Gloves ~ 10 Boxes per case / 100 per box 4. 648 cases X-large Gloves ~ 10 Boxes per case / 100 per box The following must be included in your Quote. (delivery time is as important as price): Provide non-sterile FDA approved nitrile exam gloves with an Acceptable Quality Level (AQL) or 1.5% or lower. Gloves need to be medical grade and in packaging quantities of 100 or less. Must clearly state the Manufacturer of the product you are offering. Salient Characteristics: Non-sterile/Ambidextrous Powder free - Latex-free - FDA approved - Manufactured in an ISO facility - Nitrile material - Manufactured in an ISO facility & Chemotherapy tested preferred but not necessary. (vi) Contractor shall deliver all items no later than 60 days ARO, FBO destination, from receiving the award. (vii) Provision 52.212-1, Instructions to Quoters Commercial (Oct 2015), applies to this acquisition. Addendum to FAR 52.212-1 Instructions to Quoters Commercial Items: (a): Items quoted must meet at the minimum the salient characteristics listed above. Offeror shall also provide evidence of being an Authorized Reseller (Distributor) for product offered. This can be substantiated with a link to Manufacturer s web site that supports offeror s distributorship or other proof from the manufacturer.
