Refer to the attached document that includes a list of questions with answers. A subsequent amendment is forthcoming to answer more questions. Page 3 of 3 Please provide an inventory of custodial spaces and surfaces to be cleaned. Answer: Refer to the RFQ Line Items on pages 23 through 50 and the Statement of Work (SOW), beginning on page 6 of the RFQ document for requirements. Is there a minimum staffing level required from 0730 to 1600? If so, how many FTEs are required as a minimum? Answer: The Government cannot provide staffing recommendations for contractors. Reference each line item for determining the appropriate mix of labor. The Government does not anticipate dictating the appropriate labor mix. Following award of the contract, if the contractor is not meeting the requirements, and the Government believes the labor mix is a cause or contributing factor to that, it may provide its rationale to the contractor indicating that there is insufficient labor/management to meet requirements. Is the contractor responsible for cleaning operating rooms, the emergency room, the pharmacy, etc.? Answer: Refer to the RFQ Line Items on pages 23 through 50 and the Statement of Work (SOW), beginning on page 6 of the RFQ document for requirements. Is the contractor responsible for patient room turnover cleaning? Answer: Refer to the Statement of Work (SOW), beginning on page 6 of the RFQ document for requirements. Is the contractor responsible for providing COVID-related PPE? Answer: Reference the revised SOW attached in Amendment 0001 of this RFQ, including Paragraph 6.3. Is the VA open to offerors submitting more than 3 contract references? And if so, will the VA review the additional references? Please advise. Answer: Information is provided in Paragraph 8 of FAR 52.212-1, Instructions to Offerors Commercial Items, located on page 105 of the RFQ document. The VA is specifically requesting a list for the last three contracts. Past Performance Questionnaires are required from both the COR and the Contracting Officer at each reference site.  We have been talking to CO s who say they know nothing about performance and would have pass the PPQs on to the CORS anyway. We hesitate to create non-productive burden.   Would a single reference from the COR at each reference suffice? Answer: The instructions provided at FAR 52.212-1, Instructions to Offerors Commercial Items, direct who to send the questionnaire to.   e-mail the questionnaire to both the Government contracting activity and the Contracting Officer s Representative (COR) responsible for the past/current contract. Either can complete the questionnaire. Janitorial crews are mandated to also clean equipment.  Can an inventory and location of equipment be provided to define this work component? Answer: Reference the revised SOW attached in Amendment 0001 of this RFQ, including Paragraphs 4.1 through 4.3. The sentence in Paragraph 4.1 of the SOW that reads: The contractor may perform cleaning, minor repair and other normal maintenance requirements of equipment contractor equipment on site. has been changed to: The contractor may perform cleaning, minor repair and other normal maintenance requirements of contractor equipment on site. A supervisor must be designated for each shift.  Can this be a working supervisor to maximize value? Answer: The Government cannot provide staffing recommendations for contractors. Who is the contractor currently providing janitorial service to the Phoenix VAMC  on weekends? Answer: There is not a current contract for services of the same scope as contemplated in this RFQ for janitorial services on weekends.
