Description
SPECIAL NOTICE: Intent to Sole Source Notice The Department of Veterans Affairs, VISN 23, in accordance with FAR 6.302.1 (only one responsible source and no other supplies or services will satisfy agency requirements) intends to increase contract VA263-15-D-0135 s maximum contract value/ceiling from $4.8M to $5.1M; an increase of $300,000.00 on a sole source basis with Physicians Laboratory PC. The non-personal negotiated fixed-price Indefinite Delivery Indefinite Quantity (IDIQ) services contract was awarded on a competitive basis for one base year and four (4) one-year options and a 6-month extension, FAR 52.217-8, Option to Extend Services. The 6-month extension of services runs from January 1, 2021 June 30, 2021. Performance began on January 1, 2016 under NAICS 621111 with a size standard of $11 Million. The Contractor performs off-site laboratory testing services and analysis of patient specimens. Testing includes routine and STAT analyses in pathology, cytology, autopsy testing, cytopathology, toxicology, hematology, coagulation, urinalysis, microbiology, etc. The contractor provides all supplies, equipment, materials, tools, facilities, transportation, and personnel necessary to support professional services in such quantities and state of condition as to assure continuity of said services to the facility s practitioners at a level of quality required to maintain regulatory compliance. Contractor is responsible for courier services of specimen on a scheduled basis from three(3) Nebraska-Western Iowa (NWI) VA Health Care System (HCS) facilities, Grand Island Community Based Outreach Clinic (CBOC), Lincoln CBOC and the Omaha VA to a certified laboratory testing facility for test preparation, testing, analysis, and reporting of results back to NWI VA HCS. This includes all materials necessary to collect, preserve, transport (this includes containers of various sizes for the specimens, collection containers (of such quality that specimens will not be compromised, i.e., polypropylene)/kits for both surgical and cytopathology specimen(s) immunofluorescent preservative, EM fixative, flow cytometry preservative, etc.), and process specimens examined at the contractor s facility as part of the provision of the professional services. The contractor is also responsible for maintaining their facility in regulatory compliance with respect to VA, Federal and State regulatory agencies including but not limited to: VA Office of Inspector General (OIG), Food and Drug Administration (FDA), and Centers for Medicare & Medicaid Services (CMS) as well as to accrediting entities such as but not limited to College of American Pathologists (CAP), American Association of Blood Bankers (AABB), Occupational Safety and Health Administration (OSHA) and The Joint Commission (TJC). The reference laboratory(s) must continuously hold a Certificate of Compliance or Certificate of Accreditation from the Centers for Medicare and Medicaid Services as meeting the requirements of the Clinical Laboratory Improvement Amendments (CLIA) or must demonstrate accreditation by regulatory agency(s) with deemed status from the Centers for Medicare and Medicaid Services, e.g., The College of American Pathologists, and/or other statute regulatory agencies, as appropriate, and as mandated by federal and state statutes. The reference laboratory must maintain valid certifications during the performance period of this contract. This notice of intent is not a request for competitive proposals. It is not a solicitation or request for offers. No solicitation package is available. A determination not to compete is solely within the discretion of the Government. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government.
