(i)�� �This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued.� (ii)�� �Solicitation number 75H70421Q00009 applies, and is issued as a Request for Quote (RFQ).� (iii)�� �This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-02.� (iv)�� �This requirement is a total Small Business set-aside and the associated NAICS is 541519 with small business size standard of 150 Employees. (v)�� �The Contract Line Items (CLINs) structure is as shown below. CLIN 0001: IBM Hardware and Software Maintenance Base�� � PN: IBM-MNT-HIS-2021 Period of Performance: 1/1/2021 through 12/31/2021 Total Firm-Fixed Price $_________ (vi)�� �This firm fixed price requirement is to acquire IBM hardware and software maintenance licenses for the host AIX operating system and IBM High Availability Cluster Multi-Processing (HACMP). These systems require �hardwe maintenance expertise which include hardware replacement for failed components, periodic software updates, and on site support for non user installable hardware. AIX is the common name applied to both the operating system(OS) and related hardware platforms. This platform is used to run IHS Mission critical software used in Electronic Health Care (RPMS) as well as reporting systems. (vii)�� �N/A (viii)�� �The provision at 52.212-1, Instructions to Offerors � Commercial Items (Jun 2020) applies to this acquisition.� (ix)�� �52.212-2, Evaluation -- Commercial Items (Oct 2014) The award will be made to the lowest priced technically acceptable offeror. �To be considered technically acceptable for the award, the offeror shall provide a reseller/partnership certificate or other documentation that evidences the company�s eligibility and capability to provide the requested IBM products. Additionally, the offeror shall provide the following information for two relevant federal contract awards the offeror received within past 4 years: IBM Product description, project magnitude, outcome and customer contact information. Only exact �brand name� products and part numbers shall be acceptable with the offer. No �identical� or �like� products and part numbers will be accepted. �A total firm-fixed price is being requested and price breakdown is not required. (x)�� �Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Nov 2020), with its offer. �The Offeror may complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov.
