- This is a Combined Synopsis/Solicitation is for new BRAND NAME Midmark 626 Barrier-Free Exam Tables, for our new community based outpatient center (CBOS), Blue Hen Corporate Center, Veterans Administration, Dover Delaware, as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24420Q0166 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-02 dated 11/23/20. (iv) This solicitation is set aside 100% Total Small Business Set-Aside and the associated NAICS code 339113 has a small business size standard of 750. (IV) Schedule of Requirement/CLINS (v) Statement of Work and Salient Characteristics STATEMENT OF WORK Statement of Need for: BRAND NAME Midmark 626 Barrier Free Exam Chairs Introduction The new Community Based Outpatient Center (CBOC), BLUE HEN CORPORATE CENTER, Veterans Administration Suite 3C, 655 S. Bay Road, Dover, DE 19901 has determined the need to purchase new Midmark Barrier-Free exam tables to fill the new exam rooms. The building has a tentative opening of January 2021. Justification: The project shall require 626 Midmark Barrier-Free Exam Tables to the new BLUE HEN CORPORATE CENTER CBOC location. The Wilmington VAMC shall use the white glove service, which will include delivery, unboxing, setup of the chairs directly to the exam room, and removal of all trash. If there are any delays in construction, the contractor shall hold the equipment. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
