Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpartĀ  12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C25721Q0294 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-02. The NAICS Code for this request for quotes is 621511 ($35.0 million). No set-aside is used for this solicitation. The tests being requested are listed in detail in Specialty Genetic Testing CPT. The reference laboratory testing services are required for the South Texas Veterans Healthcare System Laboratory at 7400 Merton Minter, Blvd., San Antonio, TX 78229. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria are technical acceptability and price. Under technical includes the ability to provide all of the requested items in accordance with the Statement of Work, provide a letter of authorization (if not the manufacturer) and a description of equal items, and have, at minimum, a Satisfactory past performance rating. Offerors are to provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
