Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This is a request for quotes (RFQ) under solicitation number 36C262-21-Q-0262. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-101 effective November 26, 2018, respectively. The solicitation is in accordance with FAR Part 12 and 13, Acquisition of Commercial Items, Simplified Acquisition Procedures, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. This is a SDVOSB Set-Aside Solicitation; the North American Industrial Classification System (NAICS) code for this requirement is 339112. ONLY RESPONSES FROM CERTIFIED SDVOSB FIRMS WILL BE ACCEPTED. The Department of Veterans Affairs, Veterans Integrated Services Network (VISN) 22, headquartered in Los Angeles, California has a requirement for Surgical tools. The Contractor shall provide the products to the VA Los Angeles Healthcare System. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of the contract. The Government intends to award a single award firm-fixed-price contract. The order shall begin on the date of award. **SEE ATTACHED SOLICITATION WITH ATTACHMENTS FOR INSTRUCTIONS TO OFFERORS, SCHEDULE, PERIOD OF PERFORMANCE, DETAILS OF THE REQUIREMENT, CLAUSES, PROVISIONS, TERMS AND CONDITIONS** ADDITIONAL DOCUMENTATION REQUIREMENTS: Follow ALL directions and requirements within the solicitation. Incomplete submittals may be considered nonresponsive for failure to provide a complete submittal. In order to comply with the debt collection improvement act of 1966, all contractors must be registered and have an active registration in the System for Award Management (SAM) to be considered for an award of a federal contract. This system combines data that was formerly contained in the Central Contractors Registration (CCR) and Online Representation and Certifications (ORCA). For information, review the SAM website at https://www.sam.gov. All Offerors must be registered in www.sam.gov to do business with the Government. Ensure your registration is correct, current and active.
