Description
REQUEST FOR INFORMATION An Electronic Medical Records (EMR) or Electronic Health Records (EHR) includes some important attributes across many elements of resident care making it a key, but fundamental element for delivering on excellence in resident health and safety. EMR is generally a single digital copy of a patient�s chart or series of charts associated with the discipline or source of care provided. EHR is an all-inclusive and integrated snapshot of the patient�s overall health record, designed to be shared or incorporated in a broader care plan with other providers. EHR offers streamlined sharing of updated, real-time information. EMR can be used by a physician to treat and diagnose a patient, chronicle a resident�s history for collaboration by all medical professionals who work with the individual. The terms, EHR and EMR, are often used interchangeably. Currently AFRH is technologically fractured against the need for a continuous, seamless, integrated approach in electronic health management. Each campus uses systems that comport to its own vendor/suppliers, both use Dr. Cloud and have individually requested updates/enhancements creating further inconsistencies from an Agency perspective, and Care Plans are burdened by manual operations and PDF uploads making the data and information less accessible and absent a technology solution, among other concerns. Further, the Incident reporting system within Dr. Cloud does not conform to the current industry standard of having a separate platform for this type of quality measure tracking tool that should not be a part of the Resident�s medical record. Some of the information recorded in electronic medical records includes medical history, medications, allergies, immunization status, laboratory test results, radiologic images, vital signs, the patient�s statistics (age and weight), and all insurance billing information. An EHR System should be able to help perform many tasks that help administrative and health care staff perform many of their day-to-day tasks, saving time and money. Such capabilities and qualities, and may include the following, which are some of the things identified in a series of 3 Project meetings held with AFRH Campus leadership and staff on November 9 and 10, 2020.: Immediate Access � The EMR system gives immediate access to a patient�s diagnoses, allergies, lab test results, medications, dental care, optical care, therapy, and other information that support care plans. This further allows and supports the health and care of residents. Specialty Specific � Terminology within each EMR system is specific to the care giver�s specialty, making billing and coding easier to perform for services performed by AFRH personnel to Medicare, Tricare or other third party support and reimbursement to the Home, and separate from specialty contractor support and related billings. This helps administrative medical assistants use the correct ICD-10, CPT, and HCPCS specialty billing codes so claims are not denied by insurance companies, Medicare, and Medicaid.� These codes are defined later in this document. Appointment Scheduling � EMR can help potentially help track and schedule appointments.� E-prescription management � The EMR system can write electronic prescriptions to send them directly to the pharmacy. Medical Billing Management � The EMR system can manage the physician�s or medical facility�s billing and accounting. A typical system can also interface with clearinghouses for claims submission and tracking. Charge Capture Function � The EMR system can store lists of billing codes (ICD-10, CPT, and HCPCS) for charges associated with diagnoses, procedures, supplies, and lab tests. Also, Evaluation and Management (E/M) codes are used to obtain the highest possible reimbursement allowed by law. A typical system will alert the user to an error that could lead to denial of an insurance claim. Eligibility Verification � The EMR system can perform online verification of insurance eligibility. Referral Management � The physician can keep a database of recommended referrals and share the patient�s records with others through the EMR system. Integrated Laboratory Ordering � Integrated laboratory ordering enables the ability to interact with laboratories to order and receive results for a patient�s records. Patient Interaction � The EMR system can give access to the patient to adhere with HIPAA (Health Insurance Portability and Accountability Act of 1996) regulations. It can also allow residents to set appointments, and assists residents with home monitoring and self-testing through interactive education to help improve the control of chronic conditions. Adherence to Government Regulations � The use of uniform data standards for electronic data storage allows a AFRH to comply with federal and state reporting requirements for patient safety. Electronic Medication Administration Record (eMAR)� A system that allows for medications administration and tracking within the system. Computerized Physician Order Entry (CPOE) � A System that allows the ease of providers to electronic order medications within the EMR. For more information, see complete attachment.
