- Sources Sought Notice for Ambulance Service at Dayton VAMC The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB/VOSB verified vendors who hold a current state of Ohio Emergency Medical, Fire and Transportation Services (EMFTS) certificate to perform ambulance service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation summarized as follows: Short Title: Ambulance Service at DAYTON VAMC for Basic and Advanced Life Support NAICS-621920: Ambulance Service Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition Place of Performance: Department of Veterans Affairs | Dayton VA Medical Center (VAMC), 4100 West Third Street, Dayton, Ohio 45428-0004 Anticipated Periods of Performance: 04/01/2021 to 10/31/2025 (Base plus Four Option Years) General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as specified in the attached PWS. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. Anticipated Maximum Quantities: The estimated maximum annual not-to-exceed (NTE) quantities for the requirement specified in the attached PWS are summarized as follows: BLS Ambulance BASE RATE | One-Way Trip WITHIN 20-miles DAYTON VAMC: 1,500 = Estimated maximum NTE one-way trips per year BLS Ambulance MILEAGE RATE | One-Way Trip OUTSIDE 20-miles DAYTON VAMC: 10,000 = Estimated maximum NTE miles per year BLS Ambulance WAIT TIME | 15-minute Increments IAW PWS: 15 = Estimated maximum NTE 15-minute wait-time increments per year ALS Ambulance BASE RATE | One-Way Trip WITHIN 20-miles DAYTON VAMC: 230 = Estimated maximum NTE one-way trips per year ALS Ambulance MILEAGE RATE | One-Way Trip OUTSIDE 20-miles DAYTON VAMC: 5,000 = Estimated maximum NTE miles per year ALS Ambulance WAIT TIME | 15-minute Increments IAW PWS: 10 = Estimated maximum NTE 15-minute wait-time increments per year Historical Peak Demand Estimated Workload Data: The following historical peak demand estimated workload data for PRE-COVID and COVID is being provided as a general reference which may or may not be indicative of future peak demand workload requirements.
