SOURCES SOUGHT SYNOPSIS THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201 (e). The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (hubzone, 8(a), small, small disadvantage, veteran owned small business (VOSB), service disabled veteran owned small business (SDVOSB), or large business) relative to NAICS 811219, $20.5 size standard.  Note that if VA s pending market research establishes that two or more VOSB or SDVOSB, who are certified in VA s Vendor Information Pages (VIP) site https://www.vip.vetbiz.gov/ eligible and capable, VA will set aside the underlying solicitation per 38 U.S.C. §8127.  Responses to this synopsis will be used by the Government to make appropriate acquisition decisions to fulfill this requirement. After review of all responses to this RFI, a solicitation announcement may be published on https://beta.sam.gov/, FBO and/or GSA eBuy. Response to this RFI are not considered adequate responses to a solicitation announcement. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. PARTICIPATING HEALTHCARE FACILITY: SOUTHERN ARIZONA VA HEALTHCARE SYSTEM 3601 S 6TH AVE TUCSON, AZ 85723-0001 GENERAL REQUIREMENTS: The Department of Veterans Affairs (VA), Network Contracting Office 22, VHA Regional Procurement Office - West, has a requirement to establish a procurement for maintenance and service for equipment BIOPlex 2200 at Southern Arizona VA healthcare System. SERVICE/WORK TO BE PERFORMED: The contractor will perform on-site preventive maintenance (PM) inspection on equipment/systems as provided for in the contract agreement. PMs will be accomplished in accordance with the Original Equipment Manufacturer (OEM) recommendations. Must use OEM certified parts and service engineer/technicians must be OEM trained on all equipment included in agreement. Preventive maintenance will be scheduled with a contact person as assigned by the Contracting Officer s Representative (COR), Point of Contact (POC) or designee. This PM work must be completed in the month specified during the request unless another date is agreed upon by COR or designee. Biomedical Engineering can be contacted at (520) 792-1450 x16679. All contractor personnel (without exception) must report to the Engineering Department, Bldg. 38-room A22, to sign in and receive a contractor identification badge before any work is performed and return to the same location to sign out and turn-in said badge along with the documented service report for the service performed. Toll free support. Unlimited service calls with on-site response within 24 hours (Monday to Friday 8:00 AM to 5:00 PM). All parts covered (wearables such as needles, syringes, etc. are not included). Travel and labor included. Designated Response time 48 hours unless otherwise agreed upon by COR or designee Response: All requests for repair service will be considered requests for emergency service unless a statement to the contrary is made by the COR, POC or designee. A competent service representative adequately equipped with tools, test instruments, service manuals, and repair parts will arrive at the SAVAHCS within the designated response time. The service representative may contact VA staff by phone before arrival at the SAVAHCS. Such contacts, however, will not serve to alter the requirements for arrival on-site except when the request for service is withdrawn by Biomedical Engineering. Waste Removal: The contractor is responsible to remove and properly dispose of all waste related to the services provide on-site at SAVAHCS.
