- This is a SOURCES SOUGHT notice for market research purposes to determine the availability of qualified sources capable of providing the services described below. Potential contractors are invited to provide a response via e-mail to Darlene Anderson at Darlene.Anderson4@va.gov (notice the #4 in the email address) by January 28, 2021 5:00 p.m. Pacific. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information your company considers sensitive or proprietary. Service: Community Based Outpatient Clinic (CBOC) for the Department of Veterans Affairs (VA), Puget Sound Healthcare System in the location of Chehalis, WA 98532 or Centralia, WA 98531, Lewis County, WA, near the Interstate 5 corridor. The proposed CBOC services include leasing, furnishing, equipping, and staffing one CBOC and providing Primary Care services to veterans. Mental Health services are anticipated to be provided by VA staff on-site and via telehealth in contractor-provided space. The proposed CBOC will provide services for an estimated 3,700 4,100 veteran patients. The current CBOC is operated by a contractor and is located in Chehalis, Lewis County, WA. The anticipated contract will be a re-competition for services to begin at the end of that contract. The anticipated contract period is a base year plus four (4) one-year options, with services estimated to commence on 4/1/2022. All companies wishing to do business with the U.S. Government must be registered at https://www.sam.gov/ ; there is no charge. Interested potential contractors: please provide the following information. 1) Company name, address, point of contact name, phone number, e-mail address, and DUNS #. 2) The North American Industry Classification System (NAICS) code is 621498, All Other Outpatient Care Centers; the largest a firm can be and still qualify as a small business for Federal Government programs under NAICS code 621498 is no larger than $22 million average annual receipts. Is your firm eligible for participation in a small business program (Small Business, HUBZone, Small Business 8(a), Small Disadvantaged Business, Women-Owned Small Business, Service-Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, or Other)? If yes, how should they be mitigated? 4) If your company were selected, how many calendar days would your company need after the contract award date before you could be completely set-up and begin providing services (please consider possible COVID delays)? Please include: - whether or not your company has experience setting up and operating a VA CBOC; - whether or not you have experience with current PACT (Patient Aligned Care Team) modeled clinics; - whether or not your company has experience using the capitated (per member per month) rate method; - the range your capitated rate might be in, considering 4 PACTs, providing Primary Care services for an estimated 3,700 4,100 veteran patients, with Mental Health services being anticipated to be provided by VA staff on-site and via telehealth in contractor-provided space, in the location of Chehalis, WA 98532 or Centralia, WA 98531, Lewis County, WA, near the Interstate 5 corridor, e.g., $60-$70, $70-$80, $80-$90, $90-$100, $100-$110, $110-$120, $120-$130. The capabilities statement request is not a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding your company's ability and price range to provide the services listed above. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. The capabilities will used to determine to set-aside for the Small Business community or to conduct as an Unrestricted procurement. Other than small businesses are requested to respond to this notice in the event the market does not indicate Small Business interest. 6) Provide any additional feedback that you feel is relevant (e.g., problems or any other issues experienced with similar contracts).
