- (i)�� �This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii)�� �Solicitation number 75H70421R00010 applies, and is issued as a Request for Proposal (RFP). (iii)�� �This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-05. (iv)�� �This requirement is a total Small Business set-aside and the associated NAICS is 611430 with small business size standard of $12M. (v)�� �See the attached Additional Terms and Conditions � Section B, for the pricing structure. � (vi)�� �This firm-fixed price contract is for a Health Coaching Training Pilot Program per the attached Statement of Objectives. The contract personnel is expected to work onsight at an IHS facility as well as virtually due to covid-19. See the attached Additional Terms and Conditions � Section C, for further details. � (vii)�� �N/A. (viii)�� �The provision at 52.212-1, Instructions to Offerors � Commercial Items (Oct 2018) applies to this acquisition. (ix)�� �52.212-2, Evaluation -- Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Executive Summary; 2) Relevant Past Performance; 3) Proof of Program that is NBHWC Certified; 4) Key Personnel and 4) Price. Technical Factors, when combined, are slightly more important than price. All non-price factors are of equal importance.
