Description
This is NOT a solicitation or request for quote (RFQ). This is NOT a request for proposal (RFP). This is NOT a job advertisement for hiring employees. The VA is seeking QUALIFIED VENDORS to provide a very brief response to this notice including only just what is asked for in this notice below. This is sources sought notice to industry and the only information is posted on Contract Opportunities web page at https://BETA.sam.gov/. Sources sought means seeking qualified vendors. The attachments included on the Contract Opportunities page do not include any additional information. If your company is interested please read this notice entirely and submit the required information, as requested herein. VA is seeking qualified who have interest and capability of performing transportation / courier services within Lewis and Clark, Broadwater, and Jefferson counties, for the Montana VA Health Care System, Fort Harrison, MT, on an ad hoc, as needed basis upon contact award. The requirements will be for after hours (1600-0800) and weekends for transportation of enrolled Veterans that have received or will receive care at either the Montana VHA at Fort Harrison, the Sleep Center in Helena Montana, and Elk Mountain in Clancy MT. Additionally, the contractor will provide transportation of providers from and to the Helena Regional Airport on Friday evening and Monday mornings. Laboratory results may require transportation to and from St. Peters Hospital, Helena Montana. This service will be no more than 5 times in a calendar month. Veterans requiring transportation from Fort Harrison, the Sleep Center, Elk Mountain may require transportation to their place of residence. These residencies fall within the three counties of Lewis and Clark, Broadwater and Jefferson Montana. PROVIDER TRANSPORTATION: Provide Health Care workers transportation to and from the Helena Regional Airport. The contractor will be required to provide a monthly log of services with names of the provider(s), their signature(s) and dates of transportation to the Contracting Officer Representative. VETERAN TRANSPORTATION. Provide transportation for Veterans from the Fort Harrison VHA, the Sleep Center or Elk Mountain to their home within Lewis & Clark, Broadwater and Jefferson County. This transportation will be for after hours and on weekends. This will be a on call service. The contractor will give a minimum of two hours notice for these services during the regular work week M-F and four hours on weekends and holidays. The contractor will be required to provide a monthly log of services with names of the Veteran(s), and other VHA employees, their signature(s) and dates of transportation to the Contracting Officer Representative. LABORATORY TRANSPORTATION. Provide transportation of lab tests and associated paperwork to and from St. Peters Hospital, Helena Montana and the Veterans Health Care Administration, Fort Harrison MT. This will be done after hours and on weekends only. This service will adhere to the two-hour notification in subtasks 1 & 2 and will not be required more than five times per calendar month. No log will need to be provided for this task. Services have recently been performed locally in the past using government purchase card (credit card).
