The purpose of this amendment is to: a. Answer industry questions; b. Remove ""five-year inspection"" language from the requirement; and c. Provide an updated Statement of Work (SOW). 2. The SOW is now updated with any reference to ""five-year inspection"" removed. The updated SOW attached herein MUST be the SOW submitted with any offer. 3. No other changes to the solicitation are made as a result of this amendment. KATHERINE GIBNEY CONTRACTING OFFICER Page 4 of 4 All Vendor Questions & Requests for Information Received in Response to 36C25021Q0315 When are the 5-year inspections due? Answer: There is no five-year inspection requirement for this RFQ. The five-year inspection language is now removed from the solicitation. Will the VA be supplying the qualified elevator service provider to operate the equipment and perform the testing mandated? If not, is it the responsibility of the contractor to engage another elevator service firm to perform this testing? Answer: There is a separate contract in place for maintenance and testing. This requirement is only for inspections and to make sure the elevators are being maintained properly. What was the total previous annual inspection contract value yearly and/or what was the solicitation number? Answer: The previous contract was awarded under contract number GS-21F-122BA VA251-16-F-0505. No further information will be provided as this is public information. STATEMENT OF WORK INTRODUCTION: Contractor shall provide semi-annual (every six months) elevator inspections of the electric elevators at the John D. Dingell VA Medical Center, 4646 John R. Street, Detroit, MI 48201. Elevator inspections shall be performed in the months of February and August. The purpose of the semi-annual inspections is to ensure that elevator repairs performed by contracted elevator repair service professionals are being done properly, and in accordance with federal regulations. The elevator inspections consist of (4) traction service elevators, (10) traction passenger elevators, (4) parking deck hydraulic passenger elevators, (1) hydraulic freight elevator, and (2) traction cart lifts. There are 6 elevator machine rooms to accommodate those elevators. SCOPE OF WORK: Contractor shall have all necessary tools and equipment to conduct the duties and responsibilities of each inspection of facility elevators. Contractor shall inspect all elevators and associated equipment as defined and prescribed in ASME A17.A Elevator Safety Code. Contractor shall document in detail any deficiencies found and assist in generating a proper work order(s) for the repair of any deficiencies noted during inspections. Contractor shall: Coordinate the semi-annual (every six months) inspection with the Facility Operations Specialist at (313) 576-1000, extension 65961, at least two (2) weeks in advance. Upon facility arrival, report to the Facility Operations Specialist located in room LL-636 in the lower-level of Yellow Tower. Schedule inspections on Monday through Friday business days, between the hours of 7:00 a.m. and 3:30 p.m. Inspections shall not be scheduled on any federal holiday, or any other day specifically declared by the President of the United States of America to be a national holiday. A report shall be submitted to the Facility Operational Specialist for each elevator inspection. Any safety deficiencies, or any items that require immediate attention, shall be reported by the inspector to the Facility Operations Specialist immediately. Once inspections are completed and no violations have been noted, the inspector shall sign and date the appropriate certificates for each facility elevator. If necessary, Contractor shall utilize additional, fully qualified inspectors from other regions of the United States ensure delivery of elevator inspections and testing needs, even during unanticipated peak periods. Inspection of Electric Elevators. At a minimum, the semi-annual inspection for each elevator shall include the following: Inside-Elevator-Car Inspection: Contractor shall verify safe working conditions. This includes the door opening device, stop switches, operating control devices, car floor and landing sill, car lighting, car emergency signal, etc. Machine Room Inspection: Contractor shall verify a safe work environment in all machine rooms. This includes access to machine space, headroom, lighting and receptacle, enclosure of machine space, housekeeping, ventilation, etc. Top-of-Elevator-Car Inspection: Contractor shall determine the integrity and safety of all electronic and mechanical devices on top of each elevator. This includes the top-of-car stop switch, light and outlet, operating devices, clearance and refuge space, counterweight clearance, counterweight safeties, etc. Outside Hoist-Way Inspections: Contractor shall verify compliance with federal regulations regarding the outside hoist-way of facility elevators. This includes car platform guards, hoist-way doors, vision panels, hoist-way door locking devices, access to hoist-ways, power closing of hoist-way doors, etc. Pit Inspections: Contractor shall check for safe working conditions for all facility elevator pits. Elevator inspections shall also comply with the American Society of Mechanical Engineers (ASME) A17.1, Rule 1004, Routine Inspection and Tests (i.e. compliance testing to be done every six months; verification that elevators are in good working condition). Hours of Work: Hours of work are Monday through Friday, 7:00 a.m. to 3:30 p.m., excluding federal holidays. Holidays observed by the Federal Government are New Year s Day, Birthday of Martin Luther, Jr., Washington s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. At the discretion of the President of the United States of America, any additional federal holidays may be added. Reporting Requirements: Each inspection report shall be signed and dated by the inspector and submitted to the Facility Operations Specialist within two (2) weeks of the inspection. Vendor invoice shall not be paid until the inspection report is received by the Facility Operations Specialist.
