- This is a Sources Sought only. The Department of Veterans Affairs (VA) is performing market research to determine if there are any vendors with economic status of Service Disabled Veteran Owned, Veteran Owned, Women Owned, and other Small Businesses that can provide Perfusion Services by a credentialed Autotransfusionist and equipment lease for VA Sierra Nevada Health Care System (VASNHCS). The procedure is performed approximately four to six times per year specifically for LivaNova Xtra ATS System. There is no solicitation document as this time. The NAICS is 561320 with Small Business size of $8 million. NOTE: Please ensure that System For Award Management (SAM) (www.sam.gov) indicates this NAICS code if you are interested in the requirement. Only firms interested and capable should send their information and/or capability statement and any questions by email to larry.facio@va.gov, no later than 3:00 PM Pacific Standard Time, January 28th, 2021. If interested, please provide the following information: Name of Company Point of Contact Name, phone number, and email address DUNS number Socio-economic status such as Service Disabled Veteran Owned Small Business, VOSB, Women-Owned Small Business, etc. should be included. SDVOSB/VOSB status will be verified using VetBiz. Capability statements Any unique terms and conditions required for this requirement Describe experience providing Perfusion Services and equipment List company s past experience doing business with the Federal government Price estimate of Cell Saver equipment lease, including all supplies, materials, and maintenance Identify an Hourly Price Estimate Or Per Procedure Price Estimate for the Perfusionist Services based on the requirements in the SOW. (Note, this is not a request for quote it s requesting pricing information for market research purposes only.) Before responding please carefully read and consider the following: As this is a service acquisition, the Small Business Administration and the Federal Acquisition Regulation have a Limitations on Subcontracting requirement. For all small businesses, to include SDVOSB and VOSB, At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. Weekly invoices will not be accepted. Please see the DRAFT Performance Work Statements attached for further details.
