AMENDMENT 0001 Incorrect SOW-j added correct SOW AMENDMENT 0002 Answers to Questions Submitted AMENDMENT 0003 Change date for questions to be submitted, COVID-19 update for Tower Cabs.� PLEASE SIGN AND RETURN ALL AMENDMENTS AS PART AS YOUR PACKAGE. QUESTION: Would it be possible to disclose which companies are currently or have recently provided the testing, inspection, and maintenance on the fire sprinkler, alarm, and detection systems?� ANSWER:�This will be provided to awardee QUESTION; What manufacturers are the current systems?� ANSWER:�This information will be available at the site visit QUESTION: Our team may have overlooked it in the documents provided on Beta.Sam, but do we not see where any past annual reports are provided, any device types and quantities are listed, or any system asbuilt drawings are provided in order to ascertain the types of devices and quantities per system to provide an accurate response to the solicitation. ANSWER:�That information was not required as per the SOW. we will provide the drawings once the contract is awarded.� �We do not provide this prior to award.� It is highly recommended everyone attend the site visit. QUESTION: Please confirm that the set aside is for 8(a) OR SDVOSB. The beta.sam synopsis gives the impression the contractor may need to be both while the submission of offers 1.E� indicates 8(a) or SDVOSB. ANSWER:�8(a) OR SDVOSB!!! ----------------------------------------------------------------------------------------------------------------------------------------------------- The Federal Aviation Administration (FAA) is seeking competent and qualified contractors to perform fire sprinkler, alarm and detection systems testing, inspection and recurring maintenance at the following facility in Arizona: Phoenix Sky Harbor Air Traffic Control Tower/Terminal Radar Approach Control Facility 3500 East Sky Harbor Blvd. Phoenix, AZ 85034 Below is a sample list of Protective Service Occupations services to be provided at the above locations. Inspection, Testing and Maintenance of system performance for the fire Sprinkler system and fire alarm detection systems and stairwell pressurization systems. Inspection, Testing and Maintenance of primary and ancillary equipment associated with the fire sprinkler system and fire alarm and detection system. C. Other work identified in this scope of work. The contractor will be required to provide all labor, materials, supplies, and equipment (except Government furnished) to perform this work in accordance with the statement of work provided with the SIR and with all local, state, and federal laws. Work will be done in an operational facility and the contractor shall comply with all procedures, rules and regulations of the facility authorities including limitations on equipment, allowable work hours, and other issues to ensure uninterrupted operations.� All work shall be in accordance with the plans and specifications. 1. This project is set-aside for 8(a) AND SDVOSB concerns to all Protective Service Occupations Contractors. 2. The North American Industry Classification System (NAICS) is 561621, Security Systems Services (except Locksmiths). The small business size standard is $22.0 million. 3. The project magnitude is between $50,000 and $100,000. 4. The contractor receiving the award is expected to perform at least 25% of the work utilizing their own employees. 5. Contractors must have an active registration in System Award Management (beta.SAM) before award can be made. Contractors can register at www.beta.sam.gov/. Using the Past Experience/ History form (Attachment #1), provide a listing of at least 3 past or current Facilities Maintenance contracts/projects your firm has performed in the same size ($250,000 and $750,000) and scope of this project. Contracts/projects must be either in process or within the last 5 years. Scope is described above. Similar work must include FAA Centers, Towers, or other life-critical facilities (i.e. � hospitals, schools, prisons, etc.). Offeror shall identify the contracting agency, (FAA, Other Federal, State, local governments, and private). You must provide the contract number or project identifier, which includes a point of contact and phone number.� The information provided will also be used as part of the responsibility determination. A financial capability letter from your financial institution. Name, email and telephone number of bank�s point of contact Number of year�s business has been conducted with each bank Types of open accounts (checking, loans, etc.) Balance of current accounts (the banks will provide a �range of figures� for this information, such as medium five-figures range) Complete and return the Document Security Notice to Prospective Offerors (Attachment #J-5). FAILURE TO SUBMIT ALL REQUIRED INFORMATION WILL DEEM THE REQUEST FOR SOLICITATION AS NONRESPONSIVE.
