THIS REQUEST FOR INFORMATION (RFI) 36C26221Q0316 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 334510 (size standard is 1,250 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Fresenius Parts or equal to for 2008T Dialysis Machines & Aqua that at a minimum meets the following salient characteristics in the Statement of Work for the VA San Diego Healthcare System: Item # Description/Part Number Qty Unit Unit Price Amount 0001 Year 1, See Excel Attachment for Product Description and Provide Pricing 1 LOT     1001 Year 2, See Excel Attachment for Product Description and Provide Pricing 1 LOT     2001 Year 3, See Excel Attachment for Product Description and Provide Pricing 1 LOT     3001 Year 4, See Excel Attachment for Product Description and Provide Pricing 1 LOT     4001 Year 5, See Excel Attachment for Product Description and Provide Pricing 1 LOT     TOTAL $ - Include shipping cost on the items price. Vendor Requirements: Meet the Period of Performance in the Statement of Work. Vendor shall quote all items under the Attached Excel Spreadsheet (1,482 Items) and price each line items above for the 5 Year Requirement. See P12 FRESENIUS PARTS LIST 2021 ATTACHMENT 21JAN21. Vendor shall meet the Salient Characteristics in the Statement of Work (SOW). Vendor shall respond questions 1 through 8 of the Request of Information (RFI), see below. Vendor must provide a letter showing that they are the manufacturer and/or an authorized distributor for the Fresenius products or Equal to Product. Products must be new, refurbished products will not be accepted. Vendor must meet VHA Handbook 1042.01 and VHA Directive 1053. The product in line item 6.5 must work with the current software and equipment. STATEMENT OF WORK OBJECTIVE: The Department of Veterans Affairs (VA), VA San Diego Healthcare System, has a requirement for dialysis machine and portable reverse osmosis (RO) machine parts. BACKGROUND: This requirement is a Firm Fixed Price 5 Year IDIQ contract to procure parts for the current Fresenius 2008T dialysis machines and Fresenius AquaC UNO H portable RO machines. DESCRIPTION OF SUPPLIES Vendor shall refer to the excel spreadsheet for the description of supplies PERIOD OF PERFORMANCE: Year 1 is from March 15, 2021 to March 14, 2022 Year 2 is from March 15, 2022 to March 14, 2023 Year 3 is from March 15, 2023 to March 14, 2024 Year 4 is from March 15, 2024 to March 14, 2025 Year 5 is from March 15, 2025 to March 14, 2026 DELIVERY LOCATION: (664) VA San Diego Healthcare System La Jolla Village Drive San Diego, CA 92161 SALIENT CHARACTERISTICS: The contractor shall provide the specific dialysis parts that will work with the current dialysis machines and portable machines. BLOOD PUMP MODULE Internal, custom-designed, 5 x 11 module compatible with current dialysis machine listed in section 2. Measurements have to be exact in order for this part to work. Includes integrated Start/Stop Key, Arterial Pressure Port, arterial drip chamber Level Adjust Key, blood flow rate Display Window, Blood Pump Rotor, pump speed adjustment Up/Down Keys, bloodline Tubing Retainer. Draws blood from the patient and pumps it to the dialyzer and back to the patient in a closed circuit. Pump housing with circular track for pump segment. Pump rotor has twin rollers for proper tubing occlusion Rate of the pump adjusted using the arrow keys on the blood pump or the Blood Pump Rate button on the dialysis machine Home screen. The blood pump can be stopped by pressing the Start/Stop key or by opening the blood pump door. Adjustable for pediatric and adult size blood tubing segments. HEPARIN PUMP MODULE Internal, custom-designed, 5 x 11 module compatible with current dialysis machine listed in section 2. Measurements have to be exact in order for this part to work. Includes integrated Barrel Lock Tabs, Wings Slot, Syringe Plunger, Slide Carriage & Carriage Latch Module injects heparin into the blood circuit gradually over the course of the treatment and/or as a bolus. Module can accommodate a variety of syringes that are commercially available. Module works in conjunction with the Heparin screen where such parameters as the size and type of the syringe, infusion rate, infusion time, and bolus amount of heparin to be infused are selected. LEVEL DETECTOR MODULE Internal, custom-designed, 5 x 11 module compatible with current dialysis machine listed in section 2. Measurements have to be exact in order for this part to work. Includes integrated blood chamber Level Detector Sensor, Venous Drip Chamber Holder, Optical Detector Door, Up/Down Level Adjust Button, Venous Clamp, Optical Detector, Venous Pressure Port, Chamber holder Door Latch The Level detector module is used to monitor the level of fluid in the dialysis bloodline venous drip chamber. An ultrasonic device inside the chamber holder monitors the drip chamber for the presence of air. If the level of blood in the chamber is too low and air is detected, the machine alarms, the blood pumps stops, and the clamp occludes the venous blood tubing. An optical sensor located below the occlusion clamp recognizes whether or not blood, an opaque fluid, is detected in the venous line. Also located on the front of the module is a pressure port which monitors the pressure of the venous side of the blood circuit, which is displayed in the Home screen. UI-MICS CIRCUIT BOARD ASSEMBLY WITH GEN 3 PC Internal circuit-card compatible with current dialysis machine listed in section 2. Integrates an additional function to the dialysis machine known as Clinical Data eXchange (CDX), which is an independent PC that allows for installation of a custom operating system. The UI-MICS CDX PC can run software programs that are displayed on the dialysis machine display screen and accessed with the keyboard, touchscreen, and touchpad like any other personal computer (PC). UI-MICs board Pressing the blue CDX key on the keyboard will switch between displaying Dialysis/SLED and CDX. Vendor shall only provide circuit cards and GEN 3 PC compatible with existing equipment. HYDROCHAMBER ASSEMBLY Internal, custom-designed multifunction device compatible with current dialysis machine listed in section 2. Provides for level control of reverse-osmosis water feeding into machine Activates opened/closed position of water inlet valve Provides porting and chambers needed to create calibrated negative pressure in hydraulic system. Includes integrated 110V heating rod which is used to maintain calibrated temperature control of dialysate. VENDOR RESPONSIBILITIES: Vendor is responsible for returning and replacing any parts recalled by the manufacturer for any reason at no cost to the ordering facility. The Vendor shall provide all parts and transportation required to maintain and repair the equipment, FOB origin. Vendor must provide a letter showing that they are the manufacturer and/or an authorized distributor for the Fresenius products. INSPECTION AND ACCEPTANCE Products will only be accepted if they are determined to be of acceptable quality and working order, as determined by the VA San Diego Point of Contact. MAINTENACE AND REPAIRS This requirement does not require maintenance and repairs. WARRANTY Contractor shall provide standard warranty for the items listed in paragraph 3. Product Description and Quantities. ORDERING PROCEDURES: Vendor shall have a quick turn around on the delivery for the products. Vendor shall work and schedule deliveries with the VA POC. PACKAGING AND MARKING: Vendor must adequately package parts to prevent damage during shipping and handling. All outer packages shall contain the complete contract number, obligation number and shipping address. A packing list/slip shall be enclosed with each shipment indicating the order number, items, unit of issue, items/quantity ordered and items/quantity shipped. GOVERNMENT HOLIDAYS: The Vendor is not required to provide service on the following National holidays, not shall the Vendor be paid for these holidays. The following national holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December If a holiday falls on Sunday, the following Monday shall be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States.
