Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-09. This solicitation is set-aside for SMALL BUSINESS The associated North American Industrial Classification System (NAICS) code for this procurement is 624190 with a small business size standard of $12.0 M. The FSC/PSC is G099. The VA Southern Nevada Healthcare System is seeking External Quality Review services for Delta Vet Center s Readjustment Counseling Clinicians. All interested companies shall provide quotations for the following: Services Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 Clinical Consultation Services for four (4) hours each month. 8 MONTHS OPTION YEAR ONE Clinical Consultation Services for four (4) hours each month October 01, 2021- September 30, 2022 12 MONTHS OPTION YEAR TWO Clinical Consultation Services for four (4) hours each month October 01, 2022- September 30, 2023 12 MONTHS STATEMENT OF WORK DESCRIPTION OF SERVICES Contractor shall provide External Quality Review for Delta Vet Center s Readjustment Counseling Clinicians. The contractor shall be responsible for providing quality management/quality of care reviews for the management of mental health care and continuity of care for all active veteran clinical cases. The contractor shall meet with all Clinical Staff to provide consultation feedback regarding their cases and their own professional development. Contractor shall adhere to confidentiality of patient information, and Clinician s privileged information and discussion during consultations. Contractor shall be familiar with VHA1500, Readjustment Counseling Service Guidelines, and Instructions Manual. Contractor will not have any access to VA/Vet Center Computer files or any other electronic information that may be contained in VA Computers. The contractor will strictly comply with all HIPAA requirements. 1.) The contractor will hire, train, and ensure licensure-ship of necessary personnel. 2.) The contractor will have completed sub-contracting will health care providers capable of providing primary mental health care services through the area where the plan is offered. Contractor will follow and meet the guidelines for all applicable VA and Readjustment Counseling Service policies regarding clinical treatment of veteran clients. The contractor will provide analysis and feedback to clinical staff and a written assessment in clinical chart using the established RCS recording criteria. 2. TERM OF CONTRACT This contract is for a base period of eight (8) months until 30 September and two option periods of October 01, 2021- September 30, 2022 and October 01, 2022- September 30, 2023. 3. QUALIFICATIONS Mental Health Care Providers may include licensed physicians board certified in Psychiatry or within 4 years of having completed psychiatric residency as well as nurse practitioners, physician assistants, social workers, psychologist, and clinical nurse specialist, practicing in accordance with State law and knowledgeable in the use of the DSM-V. LICENSURE OF STAFF The contractor is responsible for assuring that all persons, whether they be employees, agents, subcontractors, providers or anyone acting for or on behalf of the contractor, are properly licensed at all times under applicable state law and/or regulations. a. Physicians providing services under this contract shall be board certified in Psychiatry or within four years of having completed psychiatric residency. Physicians (including subcontractors) Physicians Assistants and Nurse Practitioners must be properly credentialed and privileged according to JCAHO requirements and VA procedures. b. Physician Assistants providing services under this contract must be a graduate of an accredited Physician Assistant program and be certified by the National Commission on Certification of Physician Assistants. Physician Assistants must hold an active, current, full and unrestricted California license and operate only within the scope of that license and acceptable community standards of practice. c. Nurse Practitioners providing services under this contract must be a graduate of an accredited Nurse Practitioner program and be certified by the American Nursing Association in either Adult Health or Family Practice. Nurse Practitioners must hold an active, current, full and unrestricted California license and operate only within the scope of that license and acceptable community standards of practice. d. Psychologists providing services under this contract must hold a current unrestricted license granted by a state/provincial licensing regulatory board and have completed Doctoral Training and a Pre-doctoral Internship approved by the American Psychological Association. e. Licensed Clinical Social Worker must hold a current unrestricted license granted by a state/provincial licensing regulatory board and have completed a master s degree in Social Work from an accredited institution. LCSW must have expertise in diagnosing and treating mental disorders in accordance with the Diagnostic and Statistical Manual of Mental Disorders (DSM-V). WORK HOURS The contractor shall provide the resources and services covered by this contract twice a month. JOINT COMMISSION ACCREDITATION OF HEALTH CARE ORGANIZATION (JCAHO) AND OTHER SPECIAL REQUIREMENTS Contractor shall have a regulatory body accreditation as required by law and will provide documentation upon request. MODIFICATIONS When necessary, the VA Contracting Officer shall make changes by written modification of the contract. PATIENT RECORDS The Delta Vet Center shall be responsible for updating and maintain all RCS patient records. 7. SECURITY Security Background Investigation: The contractor will be required to undergo any security and/or background investigation specified by VA policies and procedures and any other pertinent federal regulations. This position has been determined to be a Low Risk/Non-sensitive position. 8. EVALUATION CRITERIA Technical Expertise: All offers must submit the following to be considered for award of this contract: 1. A list of key personnel, displaying the applicable certifications and licenses required to perform these services. The list should identify personnel that will be specifically dedicated to this contract. 2. Evidence of the necessary technical capabilities and adequate resources to successfully meet the requirements in the required time. 3. Past Performance: All offers must submit at least three (3) references from contracts with a similar scope of work. References should include the organization, point of contact, phone number, and e-mail address. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Options will be evaluated at time of award The following are the decision factors: Technical Capabilities Past Performance Price The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. Addendum to 52.212-2 EVALUATION SIMPLIFIED ACQUISITION PROCEDURES Basis for Award. The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotations: Technical Past Performance Price Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation, but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following: Technical: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation. Past Performance: The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis. Price: The Government will evaluate the price by adding the total of all line item prices, including all options Total Evaluated Price will be that sum. Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by adding six months of the quoter's price for the last potential period of performance under this contract to the quoter's total price.  For example, if the contract includes option periods that are exercisable under FAR 52.217-9, the quoter's total price for the purpose of evaluation will include the base period, all option periods, and an additional six months of the last option period. Quoters must price only the base and option periods specified in the contract line item numbers (CLINs), and shall not submit a price for the potential six-month extension of services under FAR 52.217-8. The Government reserves the right to exercise the option under FAR 52.217-8 before the end of any of the contractor's performance periods and will be exercised at the price and/or rates in effect at the time the clause is exercised. Evaluation of options shall not obligate the Government to exercise the option(s). To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any.
