Page 1 of 3 Date: SERVICE STATEMENT OF WORK FOR Place of Performance: Buildings 30 and 40 Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Part 1: Project Scope of Work Overview: Contractor shall provide all tools, materials, components, labor and supervision for service to replace the frames and upgrade the operators in 6W Bed Tower and the Agent Cashier Part 2: Period of Performance (POP) and Project Construction Cost Magnitude: Complete all work including submittals, reviews and quality control within 45 calendar days from Notice to Proceed (NTP). Part 3: Detailed Project Scope of Work: The contractor shall provide the following: Task 1: Remove the doors and frames to main corridor Bed Tower Bldg 40 6W corridor, front to the passenger lobby and back to the freight elevator lobby. I.e.: two 6-foot doors set as follows: Provide hospital grade / industrial components including but not limited to wood patterned 2-Hr rated steel doors with polystyrene core, sequencer Double egress fire rated steel doors with wood veneer to match as close as possible to existing doors Electrified panic exit device Vision lites Kick plates Astragal Concealed electrified continuous (roton) hinges Magnetic lock series heavy duty electromechanical door operator in clear finish Header and back plate assembly Microprocessor controlled Obstruction shut down On-Off-Hold rocker switch Push/pull arm assembly Door mounted sensors Connected to the Wonderguard system Provide VA card access control to on both sides of the door, flush in the wall. Provide 120V from the generator power panel. Paint frame and adjacent wall to match. Task 2 The contractor shall provide the following: Remove the doors and frames to rooms 1193 and 1195: Provide hospital grade / industrial components including but not limited to steel doors with polystyrene core, no window, one-way peep hole, continuous concealed hinge, closer, closer reinforced, door sweep, kickplate Provide steel frames with mortice locks. Keyed to JB Master. Prep the jamb for electric strike. Fill the frames and provide components to meet the 2 -hour fire rating. Paint wall to match. Factory paint the doors Doors and Door Locks. The locking requirements (including access-controlled egress doors) outlined in National Fire Protection Association (NFPA) Life Safety Code standard, latest edition, 101-7.2.1.5 and 7.2.1.6 must be followed. Door Construction: Doors are of 2-hour steel construction. Dutch or half doors are unacceptable. Removable hinge pins on door exteriors must be retained with set pins or spot welded, preventing their removal. This applies only if the hinge pins are on the outside of the doors and door frames. Hinge pins will be on the outside if the door opens outward. Mechanical locking systems. Where mechanical lock systems are used, installed lock sets must allow for single motion egress. The installation of high security exit devices meeting NFPA Life Safety Code standards is appropriate. Note: Fire code prohibits locks from being locked from the inside that require a key to exit. The intent is that there must be two locks, one of which must be key operated. The other lock can be key, combination or electronic. (NFPA 101, 7.2.1.5.2 Locks if provided, shall not require the use of a key, a tool or special knowledge or effort for the operations from the egress side Doors must be set in steel frames must be fitted with a mortise lock with a deadlock feature. Per ANSIIBHMA A 156.13 American National Standards for Mortise Locks. Steel doors will not be set into wooden frames. The day lock on the main door must be automatically locking, with a minimum 19 mm (3/4 in.) dead bolt and inside thumb latch. Combinations or keys to day locks will be restricted to service employees and combinations changed immediately on the termination or reassignment of an employee. See paragraph 8 of this appendix for a detailed description of key control systems. Electronic/Magnetic locking systems. Where installed, electronic locking systems will include an automatic ""request to exit"" sensor and a ""push to exit"" manual lock release switch. Refer to the NFPA Life Safety Code for details. Part 4: Submittals and Milestone Deliverables: The Contractor Project Manager and Field Supervisor shall attend a Preconstruction Safety Meeting, to discuss the project details. SHOP DRAWINGS AND PRODUCT SUBMITTALS The Contractor shall provide all shop drawings and product submittals by email to the COR prior to ordering. Allow 14 days for VA review. Part 5: Applicable Performance Standards All work shall comply with all VA Policies including and not limited to: VA Construction Safety and VA Infection Control Policies. The Prime Contractor shall always have a Competent Supervisory Person on site when any worker(s) or sub-contractors are present. The Contractor shall provide mitigating activities as outlined in the CONSTRUCTION SAFETY PRECAUTIONS and CONSTRUCTION INFECTION CONTROL RISK ASSESSMENT / DUST CONTROL PRECAUTIONS including but not less than the following: Fire extinguishers, pedestal mounted in the area of work Walk off dust mats, inside and outside all entry / exit points to the workspace, changed not less than daily or more as required by the construction or COR. Block off all HVAC return ductwork and lovers so not to introduce dust into the HVAC system. Provide a hard-plastic dust tight temporary construction barrier to isolate the area of work. Where outside the enclosure, provide dust containment tent or portable containment cube, with HEPA filter negative air system when working above ceilings. High-Efficiency Particulate Arresting / High-Efficiency Particulate Air Filtration (HEPA) recirculating air equipment inside the workspace Seal the area of work and provide HEPA exhaust equipment to make the area of work negative air pressure with respect to the surrounding area HEPA recirculating air equipment outside the workspace at every entry / exit point No trash used packaging or construction spoils shall be stored on site. Remove each day using covered gondolas. Broom sweep the work area at the end of each day. Provide HEPA vacuum and wet mop daily where broom sweeping creates undesired dust. Part 6: Jesse Brown VAMC Additional Requirements Comply with all Jesse Brown VAMC site specific requirements and SOPs including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. Normal construction work hours are 6:00 pm Friday night through the weekend, ending Sunday midnight. Normal work hours can be modified only by written approval of the COR. No work by the Contractor on Federal Holidays and no work on the weekend when the Federal Holiday is on the respective Monday or Friday. The Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is in Patient Admitting for any work between 6:00 pm through midnight to 6:00 am or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD.
