Description
This request for information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Interested parties responding to this RFI shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. Responses are due by February 9, 2021 at 4:30pm MST. It is anticipated that a solicitation may be issued in March 2020. If issued, award of a subsequent contract is contemplated by April 2020. EMAIL INFORMATION TO: william.cornick@va.gov. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. The Phoenix Veterans Affairs Heath Care System (PVAHCS) is in need of thirty (30) FTE to support the Scheduling Operations Service. This would include twenty-eight (28) Medical Support Assistants/Administrative Professionals (DOL Labor Category: 01112 General Clerk II) to perform a variety of technical support duties that facilitate the work of physicians, physician assistants, nurses, nursing assistants and other members of the medical facility who provide direct patient care. In addition, it would include two (2) Lead Medical Support Assistants/Administrative Professionals to coordinate leave, oversee the quality of work, and act as a liaison for the contract staff. The work to be performed under this task order will be performed at the VA facility, Patient Call Center, Community Based Outpatient Clinics (CBOC), or may have the option to telework as defined by the PVACHS. Ste.105, Scottsdale, AZ 85259 (6) Thunderbird CBOC, 9424 N. 25th Ave., Phoenix, AZ 85021 MSA Contract for Scheduling Operations, PVAHCS 2237: 644-21-2-1632-0213 (7) Mid-Town Clinic, 5040 N. 15th Avenue, Phoenix, AZ 85015 (8) Southwest CBOC, 9250 W. Thomas Rd., Phoenix, AZ 85033 (1) Tours of Duty: (a) 8-hour Tours (i) 6:30am to 3:00pm (ii) 7:00am to 3:30pm (iii) 7:30am to 4:00pm (iv) 8:00am to 4:30pm (v) 8:30am to 5:00pm (vi) 9:00am to 5:30pm (b) 10-hour Tours (i) 6:30am to 5:00pm (ii) 7:00am to 5:30pm (iii) 7:30am to 6:00pm (iv) 8:00am to 6:30pm (v) 8:30am to 7:00pm The Phoenix VAHCS reserves the right to reassign the contractor based upon the facility s needs. c) WORK HOURS i) The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will provide coverage Monday through Sunday 0700-1730. Each contracted staff member is required to work a minimum of 40 hrs. per week unless on leave status. ii) Depending on the need of the facility, the tour of duty may be set at a 10-hour shift with a ½ hour lunch period or 8-hour shift with ½ hour lunch period. Two 15-minute breaks will be included in the shift. The period of performance will be for 6 months
