- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE, THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. OFFERS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This is solicitation, no. W81K0221Q0058; Purchase Request; 001160374, issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-02.� The associated North American Industrial Classification System (NAICS) code for this procurement is 621991; Size Standard for small business is $35M. �RFQ is being issued as Unrestricted.� �This requirement is for the 47th Combat Support Hospital, 62nd Medical Brigade, Joint Base, Lewis-McChord, (JBLM) FT Lewis Washington. ��The proposed purchase order is a one-time procurement for LINE ITEMS 0001 & 0002. LINE ITEM:� 0001 ��Fresh Human Cadavers�������������������������������������� ����������� 04 ����� EACH LINE ITEM:� 0002�� Prep Specialty Technician & Refrigerated Vehicle ���������� �01������ EACH����������������� ���������� ��Travel to 47th Combat Support Hospital Site SITE TRAINING LOCATION: 47TH Combat Support Hospital, FT Lewis WA, SITE TRAINING DATES: 11 March 2021 & 18 March 2021. SITE PREPARATION - PRE-ARRIVAL: 10 March 2021 - Refrigerated Vehicle with technician(s), Cadavers & Equipment to 47th Combat Support Hospital, FT Lewis WA. �������� �All responsible Contractors shall provide an offer for the following Brand Name or Equal Products that have similar, equal or better characteristics than MAXFI Surgical Simulation. Salient Characteristics & Technical Specifications The contractor must be able to provide an innovative system to perfuse cadavers, enabling life-like medical training and R & D device testing, that will provide life-like tissue integrity. This system must enable a hyper-realistic �patient� that mimics heart and circulatory functions.� This system must include a pulsatile pump, a reservoir and tubing, with an optional heat exchanger, and patented Simblood solution, which is a blood substitute.� This system overall must be able to extend the life of the cadaver over multiple days, in performing multiple procedures.� �This will be for (2) one day events, consisting of (2) cadavers for each training day a total of (4) cadavers.� �Each cadaver will have explicit consent as to meet the training requirement.� Training execution will be semi-static in nature with hospitalization and surgical training in a Combat Support Hospital scenario.� The contractor will supply, maintain and store all materials in a locked refrigerated vehicle associated with infusing and servicing of the cadavers on training site of the 47th Combat Support Hospital located at FT Lewis WA. �� EVALUATION OF FACTORS: Award will be made based on Technically Acceptable, then Lowest reasonable price.� Offeror submission must include enough information to clearly demonstrate to the Contracting Officer whether or not product services meet the salient and technical specifications listed above.� The evaluated price will be the total price of the quote, to include all fees as indicated, cold storage, freight charges, & technician escort travel as required in the solicitation. Technical Factors are more important than Price.� Price must be determined to be fair and reasonable.
