36C26021Q0301 This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below.  Potential offerors are invited to provide feedback via e-mail to Darlene Anderson at Darlene.Anderson4@va.gov .  Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Employee Stress Testing Services available within 15 miles of Department of Veterans Affairs, Southern Oregon Rehabilitation Center and Clinics (SORCC) Veteran Affairs Healthcare System, 8495 Crater Lake Hwy, White City, OR 97503. Current Procedure Terminology (CPT) codes: 93016, 93017, 93018. Stress test examination services must be available during normal business hours, Monday through Friday with evening hours. Usage will fluctuate from month to month, based on number of examinations that are due. The stress tests require: a. Review job description and functional requirements for position (Optional Form 178, pages 3 and 4) (Provided by VA SORCC), b. Review of Electrocardiogram (EKG) (Provided by VA SORCC), and c. Conduct stress test exam sufficient for determining if applicant is able to meet the functional requirements and perform the job as described without undue risk of harm to self or others. Potential contractors shall provide, at a minimum, the following information to Darlene Anderson at Darlene.Anderson4@va.gov : 1) Company name, address, point of contact name, phone number, e-mail address, and company s Dun and Bradstreet number (DUNS). All companies wishing to do business with the federal government must be registered at https://www.sam.gov/ ; there is no charge. 2) Is your firm eligible for participation in one of the following small business programs? If so, please indicate the program. Would there be an extra charge for your review of the job description and functional requirements for the position (Optional Form 178, pages 3 and 4, provided by VA SORCC)? Would there be an extra charge for your review of the Electrocardiogram (EKG) (provided by VA SORCC)? 4) The DRAFT Performance Work Statement (PWS) follows. What information do you need to know from the VA that is not included in the PWS? Do you have evening hours? 5) Provide a brief Capability Statement (less than 5 pages) with enough information to determine if your company can meet the requirement. The Capabilities Statement for this Sources Sought is not expected to be a Request for Quotations, Request for Proposals, or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding your company s ability to provide the services listed above, as needed, within 15 miles of VA Southern Oregon Rehabilitation Center and Clinics (SORCC) Veteran Affairs Healthcare System, located in White City, Oregon. Any commercial brochures or currently existing marketing material may also be submitted with the Capabilities Statement. Submission of Capabilities Statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This Sources Sought Notice is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by February 23, 2021.
