- This is a sources sought notice; this is neither a solicitation nor a promise of future work. The Altoona VAMC is seeking SDVOSB/VOSB and any Small or Large company regardless of Socio-economic category that can provide the requirement as spelled out in the work requirements below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have given you permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? Responses shall be submitted electronically, via email to Donna Toliver. Email address is donna.toliver3@va.gov. Responses are due no later than Wednesday 2/10/2021 at 3:00 pm. This requirement is for the purchase, delivery, installation, of equipment as stated below (Brand Name or Equal) at the Altoona VAMC. WORK REQUIREMENTS The vendor: Shall provide the eye lanes and all the accessories associated with the eye lanes to the Altoona VA Medical Center located at 2907 Pleasant Valley Blvd, Altoona, PA 16602. Salient Characteristics Must have all the following (brand name Reliance | Haag-Streit, USA OR equal to): 2 ea Reliance 6200 chair, low 2 ea Reliance 7900 IC/wc stand 2 ea Small wheelchair glide 2 ea CV-5000S W/KB50S Refraction System; 13081K1 PROMO: Receive a free PC 50S LCD High Resolution visual acuity chart 1 ea Topcon KR800 autoref/k with table 1 ea Topcon CL300 autolensmeter 1 ea Blue tooth adaptors to link instruments 2 ea Topcon SL-D301 Slit Lamp - LED with R tonometer 4 ea Reliance 4246 stools, 2 ea Heine Omega 500 Wireless 2 ea Heine Hand held sets with ret., direct, and transillum. 2 ea Topcon manual lensmeter 2 ea Trial lens sets with frame 2 ea First surface mirror for above, if needed, for charts Freight must be included Salient Characteristics of the Computerized Phoropter system (equal or better than CV5000S) This Computerized Phoropter platform must incorporate the following features: 1. Computerized system to replace traditional manual refraction which includes a compact optical Phoropter with sealed electronic lens banks, high-speed lens rotation, LED illumination for near testing, one dial controller and choice of charts. Benefit: Maintenance free, high-speed refraction system providing a thorough and comprehensive refraction experience 2. Tilting optical head and LED Illumination simulates natural reading angle and working distance. Benefit: User can observe patient s expression and comfort level during near vision testing at a naturally tilted reading/working angle and make changes as needed 3. One Dial Controller (KB-50S) with a 10.4 inch Color LCD Touch Screen and intuitive navigation buttons. System includes help function with step-by-step instructions. Benefit: Negates repetitive hand/shoulder motion for the user and increases user-patient social distancing 4. User can add up to 20 custom images/charts and two movies for display on Topcon PC-50S or Topcon MC series Charts. Chart selection is operated through five pages on the LCD touch screen which are customizable. Benefit: Custom charts, fixation movies/targets and day/nighttime driving simulations for both adults and children (including illiterate) 5. Pre-test Connectivity (Topcon or Non-Topcon) enables seamless interface with vision testing instruments as well as EHR. One designated pre-test room links to multiple Phoropters. Ability to enter patient name or ID into pre-test devices for easy patient retrieval. Benefit: Provides a personalized starting point for vision testing and reduces transcription errors. Sends final manifest to EHR with one touch of a button 6. Program up to 5 user-defined personalized refraction courses with built-in scripts. Benefit: Reduce exam time, improve consistency and maximize flexibility for multiple users 7. Simultaneous Cross Cylinder (Topcon Cross CylinderTM) presents red and green-framed images simultaneously in a split-screen. Benefit: Presents both options simultaneously, allowing the patient to blink normally during the refraction and arrive at a more precise, less confusing and confident result than with traditional and repetitive lens flipping techniques 8. Standard Color-Coded Jackson Cross Cylinder (JCC) with one touch lens banks flipping either for +/- refraction techniques. Benefit: MD/OD preferential workflow methods and less confusion for the patient 9. Glare, Contrast, Photopic, and Scotopic examination scenario capability. Benefit: Ability to test individually recorded exams under day/night, glare and contrast environments enabling operator to prescribe both day and night lenses for patients with nighttime myopia, hyperopia or nighttime driving difficulties 10. 21 Point Refraction test on board with full documentation capabilities. Benefit: Results can be printed and exported to EHR 11. Rx Comparison and Patient Education graphics and animations built into the computerized system Benefit: Instantly compare the old vs. new Rx. Educate the patient on the anatomy and visual conditions of the eye 12. Remote Controlled Refraction and social distancing capabilities as an option. Benefit: a remote refraction connectivity system built to facilitate refractions performed by remote clinicians and/or technicians. Increases eyecare access to remote areas without the need for onsite clinicians 13.
