- COMBINED SYNOPSIS/SOLICITATION DOCK LEVELERS AND ROLLING DOOR PREVENTIVE MAINTENANCE AND REPAIRS AT THE NEW YORK HARBOR HEALTHCARE SYSTEM, MANHATTAN CAMPUS This amendment is to advise interested vendors that a site visit has been scheduled for Wednesday, February 17, 2021. If interested, please email the Contract Specialist for further details. The last date for questions has been changed from 4:00PM EST, February 11, 2021 to 4:00PM EST, February 19, 2021. The closing date has been changed from 10:00 AM EST, February 18, 2021 to February 25, 2021 (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items and FAR part 13 - Simplified Acquisition Procedures , as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number 36C24221Q0477 is issued as a request for quote (RFQ) and will be submitted on through https://beta.sam.gov/. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1. (iv) This procurement is being issued as a Veteran Owned Small Business set aside Open Market Competition. The North American Industry Classification System (NAICS) code is 811310, Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance with a small business size standard of $8 million. (v) The Contractor shall provide pricing for Dock Levelers and Rolling Door Preventive Maintenance and Repairs. See table below. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Proposal: a) Demonstrate an understanding and acceptance of the scope of work and RFP requirements. b) Demonstrate the ability to meet the requirements of the scope of work. Team Arrangements & Subcontracting Plan: a) Provide Resumes of Key Personnel b) Provide Information Flow and Delegation of Authority and Responsibility to execute maintenance tasks Sub- Contractors: a) The sub-contractor will identify his Key Subcontractors Quality Control Plan: a) VA Engineering will evaluate the offeror s ability to provide services that met the Hospital s quality requirements. The Offeror s performance will be examined to determine whether the quality of service necessary to provide conformance to contract requirements was met. Contractor s Capabilities: Contractor will have resources (employees, supervisors, material and equipment) dedicated specifically to the performance of this contract. A P.O.C. will be at each site Five (5) days a week for Eight (8) hours a day. 2) Past Performance: a) The contractor perform work that is the same or similar in nature, size, and complexity to the services being procured under this Solicitation. b) Recent Past Performance c) Relevant Post Performance d) Past Performance Confidence Rating 3) Price: Will be determined as fair and reasonable per FAR part 13.106-3. The government intends to award a firm fixed price contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein.
