Description

Amendment 2 The purpose of this amendment is to clarify a requirement in the finish schedule.� See attached amendment. -----End Amendment Notice----- Amendment 1 The purposed of this amendment is to: a.� Post answers for RFIs. b.� Make available the site visit sign in sheet. c.� All other terms and conditions remain unchanged. See attachments for details. -----End Amendment Notice--- Project: 629-18-127, Remodel Pharmacy for USP 800 The contractor shall provide all supervision, labor, material, tools and equipment to remodel the current Pharmacy compounding area to meet USP 800 requirements; build out a new Segregated Compounding Area (SCA) as a backup to the primary compounding area at Southeast Louisiana Veterans Health Care System 2400 Canal St New Orleans, LA 70119. This procurement is 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) under Public Law (PL) 109-461. Offerors must be verified as a SDVOSB in the VetBiz Vendor Information Pages (VIP) at http://www.vetbiz.va.gov.� Offers received from contractors who are not SDVOSB verified in VetBiz will not be considered for award. Federal Acquistion Regulations require that federal contractors register in the System For Award Management (SAM) database at https://www.sam.gov and enter all mandatory information into the system. Award cannot be made until the contractor has registered. Offerors are encouraged to ensure that they are registered in SAM prior to submitting a proposal. MAGNITUDE OF CONSTRUCTION PROJECT: In accordance with FAR 36.204 and VAAR 836.204 DISCLOSURE OF THE MAGNITUDE OF CONSTRUCTION PROJECTS, the above described project falls between the following price range: Between $1,000,000 and $5,000,000.
