- Introduction: This is a SOURCES SOUGHT SYNOPSIS Notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations, and no contract shall be awarded from this synopsis. The purpose of this notice is to obtain information regarding the availability and capability of qualified sources. Your responses to the information requested will assist the Government in determining the appropriate acquisition strategy. �Background: The Indian Health Service (IHS), an agency within the Department of Health and Human Services, is responsible for providing federal healthcare services to American Indians and Alaska Natives. The provision of healthcare services to members of federally-recognized tribes grew out of the special government-to-government relationship between the federal government and Indian tribes. The Indian Health Service (IHS) is the principal federal health care provider and health advocate for Indian people and its goal is to raise their health status to the highest possible level. The IHS provides a comprehensive health service delivery system for approximately 1.9 million American Indians and Alaska Natives who belong to 564 federally recognized tribes in 35 states.� The Northern Navajo Medical Center (NNMC) is a Navajo Area IHS facility. �General Requirements: Purpose: The contractor shall provide an on-site Magnetic Resonance Imaging (MRI) fully equipped mobile trailer, including equipment maintenance, and 2 weeks of instrument training, in a contractor-owned mobile MRI unit. �Scope: The mobile unit shall be brought on-site to NNMC located at Highway 491 North, Shiprock, NM 87420 for 5 days per week for the term of the contract. Number of daily scans is estimated at 10 scans per day. �The Contractor shall maintain a sufficiently large service network to satisfy a 98% up time guarantee.� Duties and responsibilities of the Contractor shall encompass outpatient and inpatient services to IHS patients. Duties and Responsibilities: The duties include but are not limited to 24/7 maintenance coverage of mobile MRI trailer and equipment, NNMC staff training on scanner and trailer components as well as 2 weeks of training on MRI scanner and coils used. Duration of requirement: One twelve month base period, plus 4 twelve month option periods.�� �Place of Performance: Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 Instructions to Industry: All capable parties are encouraged to respond. Responses must directly demonstrate the company�s capability, experience, and ability to marshal resources to effectively and efficiently perform the objectives described above. Generic capability statements are not sufficient and will not be considered compliant with the requirements of this notice. The Government requests interested parties submit a written response to this notice which includes: - Company Name. - Company DUNS number. - Company point of contact, mailing address, and telephone and fax number(s), and website ��������� address - Name, telephone number, and e-mail address of a company point of contact who has the ��������� authority and knowledge to clarify responses with government representatives. - Date submitted. - Applicable company GSA Schedule number or other available procurement vehicle. - Business Size Standard of Company (i.e., small business, 8(a), woman owned, veteran ��������� owned, etc.) as validated via the System for Award Management (SAM) registration ����� status. - All respondents must register on the SAM website located at http://www.sam.gov/. - Geographic Coverage: Please identify the areas of the United States where your organization �� provides these services. References: Provide a list of�all private industry or government contracts for similar services that you have performed during calendar years 2017�through 2020. Please include the customers� contact names, addresses, telephone number, dollar value of contract, and brief description of the services provided on the contract. Disclaimer and Important Notes: � This notice does not obligate the Government to award contract. Any information provided by industry to the Government as a result of this sources sought synopsis is strictly voluntary. Responses will not be returned. No entitlements to payment of direct or indirect costs or charges to the Government will arise as a result of contractor submission of responses, or the Government's use of such information or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization�s qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a solicitation may be published on a government GPE. However, responses to this notice will not be considered adequate responses to a solicitation. � Confidentiality: � No proprietary, classified, confidential, or sensitive information should be included in your response.
