REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT 36C10G21Q0028 Service Dog Health Insurance Covering Wellness and Sick/Injured Care The Department of Veteran Affairs (VA), Strategic Acquisition Center (SAC) on behalf of Veterans Health Administration (VHA) (VA s Prosthetics and Sensory Aids Department (PSAS)) is conducting a Request for Information (RFI) in accordance with Federal Acquisition Regulation (FAR) 15.201(e) to determine the availability and technical capabilities of qualified sources to provide health/wellness and sick/ injured insurance for Service Dogs. In accordance with FAR 15.201 (e) responses to this notice are not offers and cannot and will not be accepted by the Government to form a binding contract. This notice does not constitute a Request for Proposals (RFP), Request for Quotes (RFQ), Invitation for Bids (IFB), or any other solicitation vehicle. Product Service Code Q- Medical services North American Industry Classification System (NAICS) Code 524128 Other Direct Insurance (except Life, Health, and Medical) Carriers. (Pet Health Insurance Carriers, Direct) Specifically, the services are to provide both (1) comprehensive wellness coverage and (2) sick/injured insurance coverage for all trained and certified Service Dogs regardless of age, breed, geographic location or pre-existing medical condition(s). See the Attachment DRAFT Statement of Work (SOW) for further details. The information provided herein is subject to change and in no way binds the Government to solicit for, or award, a competitive contract nor does it restrict the Government as to the ultimate acquisition approach. VENDOR SUBMISSION INSTRUCTIONS Companies are encouraged to respond if they have the capability and capacity to provide the services as outlined in the Attachment DRAFT SOW. Be advised that generic capability statements are not sufficient for effective analysis of respondents' capacity and capability to provide the required services. If your firm has an interest in responding to the requirement described above, please provide the following: A. Corporate Capability Statement - demonstrates capabilities, knowledge and experience related to the services as described in the Attachment DRAFT SOW. Any significant subcontracting or teaming arrangements that are anticipated for this effort should be addressed. In the event of any subcontracting or teaming arrangements, address the administrative and management structure of such arrangements. B. Responses shall be identified with: RFI number, 36C10G21Q0028 Company s name Mailing address Cage Code, Data Universal Numbering System (DUNS), and Tax ID# Applicable business Size and Status Socioeconomic category, if applicable Federal Supply Schedule Contract Number including SIN category, if applicable E-mail address Telephone and fax numbers Website address (if available) Point of contact having the authority and knowledge to address/clarify responses with Government representatives. Does your company have a system to directly reimburse veterinarians, or would your company be willing to establish such a system? Does your company have the capability of providing coverage for greater than 2,000 dogs? Interested parties shall submit a Corporate Capability Statement not exceeding 15 single-sided, type-written pages using Arial 12-point type or larger. The submission shall be clearly indexed and logically assembled. Any comments in reference to the Attachment DRAFT SOW should state the page number, paragraph number, and task number associated with the comment. The page limit does not apply to advertising material such as company brochures and catalogs. If there is any information you deem proprietary, please mark accordingly. SUBMISSIONS ARE DUE no later than March 4, 2021 at 3:00 PM Eastern Time. All responses under this RFI shall be emailed to lawrence.toperoff@va.gov and Zachary.wilcox@va.gov. Due to VA email file size restrictions (6 MB), vendors may need to logically separate their responses into separate emails. The VA email system will not accept ZIP files. If a ZIP file is sent, they are removed and not delivered. Acknowledgement of receipt of responses will not be made, nor will respondents be notified of the SAC review of the information received. Do not send any material that requires a non-disclosure agreement and identify information that is business sensitive. Responses to this notice will not be returned. Telephone calls/questions or inquiries will not be accepted.
