- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C25721Q0363 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-04. The NAICS Code for this request for quotes is 621511 ($35.0 million). No set-aside is used for this solicitation. The items being requested are as follows: Item Number Unit Description Quantity 0001 Mo Provide send out reference laboratory testing not performed in-house at VANTHCS' Pathology & Laboratory Medicine Service. Contractor will provide all labor, supplies, equipment, insurance, and supervision to provide the items. See ""Attachment A NTX Specialty Reference Lab CPT"" for ESTIMATED QTY of tests required annually. 66 This is a Single Award, Firm-Fixed Price, Indefinite Delivery Indefinite Quantity (IDIQ) Contract. The guaranteed minimum award amount for this contract is $10,000.00. The maximum aggregate value of orders that can be placed under this contract is $6,000,000.00. The Government does not guarantee that it will place any orders under this contract in excess of the guaranteed minimum award amount. Ordering Period: POP Begin: 04/01/2021 POP End: 03/31/2026 The services being requested are for the reference laboratory testing of patient specimen. Services are to be performed at the VA North Texas Healthcare System, Laboratory, 4500 S. Lancaster Rd., Dallas, TX 75216. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation factors are technical acceptability, past performance, and price. See Addendum to FAR 52.212-2 in RFQ 36C25721Q0363 .
