H (Isolation gowns levels 2-4) COVID-19 Authority; Please read the following description and attached solicitation instructions carefully for full consideration of offers. SOLICITATION NOTICE OF TIERED EVALUATIONS This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127. TIERED EVALUATIONS INCLUDING LARGE BUSINESS CONCERNS: This solicitation is being issued as tiered evaluation with the following tiers: (1) SDVOSB concerns, (2) VOSB concerns, (3) small business concerns with HUBZone small business concerns and 8(a) participants having priority; and (4) large business concerns. Description The Department of Veterans Affairs, Veterans Health Administration, New England Healthcare System VISN 1, requires PPE, (gowns, masks, gloves), syringes, and needles under the authority of� the Presidential COVID-19 emergency declaration under the Robert T. Stafford Disaster Relief and Emergency Assistance Act.� Syringes and needles line items are solicited on a brand name only basis per the Salient Characteristics attached. Deliveries may be required to any facility in the New England Healthcare System VISN 1 area which includes the States of Maine, Massachusetts, Rhode Island, Connecticut, New Hampshire, and Vermont. This solicitation will remain open and continuous following posting. Quotes will be accepted for a period commencing with the issuance date of the solicitation until expiration of the COVID-19 emergency authority, allocated funds have been exhausted, or until one year from the date of solicitation. Interested offerors will be responsible for downloading copies of the solicitation (and any subsequent amendments) from the SAM website, and no hard copies will be made available. In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VIP database if applicable (VIP registration is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at https://www.sam.gov and Vet Biz information at http://www.vip.vetbiz.gov. To be eligible for award, contractors must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website as no award can be awarded to any offeror not registered in SAM. NAICS Code is 33913 (Medical and Surgical Instruments, Equipment, and Supplies) with a Small Business Size Standard of 750 employees. The product service code is 6515. This solicitation is being issued as an open and continuous tiered evaluation with the following tiers: (1) SDVOSB concerns, (2) VOSB concerns, (3) small business concerns with HUBZone small business concerns and 8(a) participants having priority; and (4) large business concerns. � Tiered evaluation is to occur on a weekly basis each week following solicitation. Tiered evaluation will occur for each line item solicited.� If award cannot be made, the solicitation will remain open for quotes, and repeated tiered evaluation each week thereafter during the open period of solicitation. � Award quantities will be based upon weekly evaluation of VISN 1 facility Line Item supply gaps monitored by VISN 1 Logistics and supplied to the Administrative Contracting Officer each Wednesday following solicitation.� Gaps in supply for the individual line items are identified according to a 120-day supply goal, and a rolling 30-day average usage model. Tiered evaluation will occur only for qualified quotes in hand at 4:30 p.m. Eastern Standard Time each Wednesday following the solicitation date. Tiered evaluation will occur under the evaluation criteria enclosed, and according to tiered evaluation authorized under VHAPM Part 819.7004 TIERED/CASCADING SOP, and Class Deviation from Federal Acquisition Regulation 19.502-2, Total small business set-asides (VAIQ 7867323), dated March 22, 2018.� Multiple and single awards are anticipated and will be made until identified gaps in individual line items are satisfied. All responsible sources may submit proposals to be considered by the Agency. VA will only consider proposals from offerors able to represent that they are authorized distributors of the supplies for which they are providing quotes.� Acceptable documentation will be letterhead communication from the manufacturer, identifying the offeror in full, and providing contact information for verification purposes. Authorized Distributorship is subject to verification. Proposals must be for quantities held by the offeror within the Continental United States, and available for immediate shipment.� All proposals will be subject to technical acceptance according to the salient characteristics. Offerors must identify the products offered by Manufacturer, Country of Origin, and Make or Model Number.�� Offerors may be required to provide samples of offered products for technical evaluation.� Proposals for singular and multiple combinations of the listed Line Items WILL be considered. Proposals must identify any applicable expiration for the offer.� Proposals will be considered under the following weeks tiered evaluations until the identified expiration, or until retracted by the offeror. Substitutions of product or pricing will NOT be accepted under any week�s tiered evaluation. Additional quotes received from an offeror will be considered under the period of evaluation that they are received.� Ambiguous offers will NOT be considered.
