- RESPONSE (CAPABILITY) To assist the Government in identifying potential sources, the respondents will need to provide documentation that sufficiently addresses their organization s qualifications, ability to perform as a prime contractor for the services required. Documentation will need to include the following: Company name, address, and DUNS number; Company point of contact to include phone, fax, and e-mail information; Company s socioeconomic status (e.g. small business, service disabled veteran owned small business, large business) Contract numbers, along with points of contact for the contracting officer and contracting officer s representative of recent (within the last 24 months) contracts you have held of the same or similar size and scope as what is presented in this sources sought notice; The questions on the following page must be answered and submitted with your response. All interested offerors should submit this information by email to: nicole.gregory2@va.gov by 3:00pm Friday, February 26, 2021. All information received in response to this RFI/Sources Sought that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this RFI/Sources Sought. Questions: National CMOP Hazardous Waste Contract Does your company have experience providing hazardous and non-hazardous waste removal services? Does your company have the resources to cover multiple facilities located throughout the Continental United States simultaneously? The specific locations are Leavenworth, KS; Chelmsford, MA; Tucson, AZ; Lancaster, TX; Murfreesboro, TN; Hines, IL; and North Charleston, SC. Does your company have the appropriate licenses and permits to perform work in the locations listed in question two? Does your company have personnel with experience and certification in handling of infectious materials and emergency procedures to be followed in case of a spill? Is your company capable of providing UN and DOT approved containers for the VA to use in secure storage areas at multiple facilities located throughout the Continental United States? Does your company have experience testing and characterizing waste then providing documentation of the result and process followed? Does your company have experience disposing of waste that contains sensitive information, such as protected health information? Does your company have personnel with 40-hour HAZWOPER certification that will perform on-site services? Does your company have experience providing tracking documents, such as uniform manifests, as well as treatment and disposal documentation? Does your company have the capability to provide on-site annual hazardous waste training to locations, listed in question two, for employees working in satellite accumulation areas? The training will cover RCRA, state specific regulations, records management, and emergency responsibilities as well as DOT and IATA. Does your company have a current registration in SAM? Does your company have a current registration in VIP?
